Job Description


Plans, directs, manages and coordinates patient care and staff activities on assigned unit to maintain standards of patient care with 24 hour accountability.  Communicates to medical staff, nursing department, administration and other disciplines in matters related to nursing service by performing the following duties personally or through supporting staff.

Essential Duties and Responsibilities include the following. Other duties may be assigned.


Supervisory Responsibilities                                              

Carries out supervisory responsibilities in accordance with CJE’s policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems; maintaining relations across Agency departments, functions, and committees as well as community organizations and the CJE Board.


  • Ensures timely and effective response to resident and family requests and inquiries. 


Specific Responsibilities Applicable to Clinical Nurse Manager, Lieberman Center for Health and Rehabilitation


  • Provides supervision and support to Lieberman nursing staff, ensuring proper, effective efficient delivery of services, appropriate staffing levels and distribution of work, and that staff is skilled and knowledgeable in service delivery, processes, and operations.
  • Has knowledge of RN Staff position, is able to assess staff nurse skills and offer guidance as needed.
  • Ensure that programs and services meet highest quality standards of nursing practice and care.
  • Is sensitive to and ensures all urgent care/emergency care needs are met.
  • Collaborates with hospitals, physicians, and all appropriate health professionals and family members to ensure efficient and quality service delivery in resident care.



  • Coordinates administrative and nursing clinical care activities during tour of duty, assuming 24 hour accountability for his/her assigned floor.
  • Directs and coordinates all nursing personnel for his/her assigned unit with the authority to act from the Director of Health Services and the Assistant Director of Health Services.
  • Provides leadership and direction to staff in the interpretation of facility's policies and procedures to assure quality resident care.
  • Provides the necessary tools so staff can effectively perform their duties and can provide quality services to residents and family.
  • Maintains close working relationship with shift supervisors as it relates to resident and personnel issues.
  • Assists in maintaining a spiritual, physical, psychological and social environment conducive to the best interests and well-being of residents and families.



  • Responsible for the completion of new admission and readmission assessments according to regulations including RAI.
  • Assesses, reports, communicates and records any change in resident condition during his/her tour of duty to appropriate persons.  Uses computer system to enter, update, and access resident data.
  • Responsible for ensuring residents attending physicians are notified when change in condition occurs or other situation warrants notification.
  • Communicates to other team members verbally or through written reports (24 hour shift report, census, etc.) about current resident status.
  • Uses clinical judgment to ensure that resident receives appropriate care during and after transfer to a hospital or other outside health providers.
  • Ensures that resident is seen by appropriate consultant and has knowledge of resident’s condition that warrants him/her to be seen.


  • Assumes a leadership role as a member of an interdisciplinary team as it relates to resident care.
  • Formulates resident plan of care after completing the initial assessment, resident assessment protocols, etc.
  • Attends care conference, integrates pertinent information into Plan of Care and educates staff.
  • Initiates need for additional resident/family/staff conferences and reports to group identified problems of the resident.  Revises or adds to Plan of Care as necessary.
  • Attends resident/family conferences and reports to group identified problems or needs of the residents and revises Plan of Care as necessary.
  • Uses clinical judgment in review of Physician Order sheets to determine appropriateness of medications, treatments and appliances.  Discontinues unnecessary treatments and medications.
  • Ensures care plans are updated to reflect pertinent information and that this is communicated to appropriate persons.


  • Makes daily rounds on assigned residents to assure plan of care is being followed.
  • Collaborates with attending and consulting physicians to assure residents’ needs are met.



  • Modifies Plan of Care to accurately reflect a residents problems or needs post hospitalization and/or change in condition.
  • Collaborates with caregivers in other agencies who are attending to his/her assigned residents in order to assure quality resident care.
  • Responsible for ensuring documentation is timely, reflects current status of assigned residents and meets regulatory requirements.
  • Does significant change of status assessment as needed and coordinates this with appropriate staff.


  • Receives and processes staff call-ins. Adjust unit assignments as necessary using established staffing patterns as developed by Director/Assistant Director of Nursing and daily resident acuity and census.
  • Supervises assigned staff Nurses and C.N.A.s in their specific responsibilities, such as documentation and professional duties.
  • Evaluates employees work performance through probationary and annual reviews.  Consults with shift supervisor concerning work performance on their specific shift.
  • Interprets work rules to employees and follows appropriate disciplinary steps.  Has authority to take disciplinary action with the advisement of the Director/Assistant Director of Nursing.
  • Conducts regular staff meetings.
  • Responsible for orientation and in-servicing of nursing personnel including outside agency (policies, procedures, issues, changes, etc.)
  • Knowledge of the budgetary process and advises Director of Nursing of staff and equipment needs.
  • Ensures distribution and proper use of supplies by assigned staff.
  • Assists with staff nurse duties when necessary.




  • Has working knowledge of the union contract and will discipline staff under the advisement of the Assistant Director of Nursing/Director of Nursing
  • May be asked to assume responsibility as the shift supervisor.
  • May be asked to serve and/or chair certain committees as it relates to resident care.
  • Reviews Policy and Procedure manual periodically.  May be asked to revise and rewrite procedures as necessary.
  • Responsible for interpreting changes and/or new policies and procedures to assigned staff.
  • Knowledge of Lieberman’s Administrative policies concerning operation of the overall facility, and the actions to be taken during emergencies or mechanical failures.
  • Knowledge of rules and regulations governing the facility and implements same under the direction of Director of Nursing.
  • Review residents’ needs with the case manager during the IDPH Inspection of Care process.  Provides inspector with information during inspection.
  • Makes decisions about resident and staff issues in collaboration with shift supervisors.
  • Ensures physician compliance concerning resident records in accordance with regulatory agencies.
  • Responsible for other duties as assigned by the Assistant Director of Nursing/Director of Nursing.

Job Requirements

Education and/or Experience   

  • Graduate from an accredited School of Nursing.
  • One or more years of experience in geriatric or acute care.
  • One or more years of supervisory experience preferred.
  • Flexible schedule required.  Required to assume Administrator on Call and House Supervisor responsibilities according to schedule.
  • Computer skills including knowledge of MS Office.
  • Strong organizational, interpersonal and communication skills required.
  • Some local travel may be required.


Language Skills                                           

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.  Ability to write reports, business correspondence, and procedure manuals.  Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Certificates, Licenses, Registrations

Must be currently licensed to practice as a Registered Nurse in Illinois.
