The Nutritionist II will conduct nutrition screening, assessment and counseling a diverse population on various nutrition related diseases such as hypertension, high cholesterol, obesity, diabetes, as well as implement nutrition care plans patients with diabetes, pre-diabetes, gestational diabetes and other high risk pregnancies, hypertension, obesity and hyperlipidemia. Assist in curriculum, educational, development and implementation of material development, teaching classes and quality improvement work.

Duties and Responsibilities:

  1. Conducts nutrition screening, assessment and counseling, and implements nutrition care plans for low and high risk patients with diabetes, pre-diabetes, gestational diabetes, and other high risk pregnancies, hypertension, obesity, hyperlipidemia.
  2. Provides individual nutrition and health counseling for those with overweight, diabetes, hypertension and dyslipidemia.
  3. Maintains documentation by charting SOAP notes in the progress notes.
  4. May develop and participation group nutrition education programs and medical group visits (i.e. diabetes, child obesity, dyslipidemia, hypertension).
  5. Collects and reviews material for education such as pamphlets, audio-visual aids, books and handouts. May assist in the development and evaluation of culturally relevant nutrition education materials.
  6. Assists in data entry and retrieval and other quality improvement efforts for the department.
  7. Promotes site based employee wellness efforts.
  8. May provide in-service nutrition/health education to staff and community partners.
  9. Participates in meetings related to nutrition, clinical health education and wellness as required/assigned.
  10. Other duties as assigned by supervisor.

Job Requirements


  1. Knowledge of contemporary principles and practices of nutrition, nutritional value of foods, interpretation and implementation of special diets, and meal planning and preparation that is culturally appropriate.
  2. Knowledge and experience with Motivational Interviewing, Stages of Change, Division of Responsibilities.
  3. Interpretation of anthropometric, biochemical, clinical, and dietary information, and determination of appropriate dietary advice that is culturally appropriate.
  4. Knowledge of aspects of storing and/or preparing foods properly; interpretation and implementation of special diets; scopes functions and responsibilities of local health departments.
  5. Ability to develop, implement, and evaluate nutrition education activities and coordinate them with other departmental programs
  6. Bilingual English/Spanish required.

Experience Required

  • Requires completion of Dietetic and Food Administration Program from an accredited university; RD or RD eligible, plus one year experience working in a community health care setting. Masters in Nutrition/Dietetics preferred.
