Job Description

POSITION SUMMARY
Under supervision of Program Management, performs duties in support of therapeutic services and provides comprehensive direct support services which are structured to meet the needs of an adult population composed of SMI, dual-diagnosed and/or referred persons. Engages clients in skills training designed to enhance functioning leading to community integration and self-sufficiency in living, learning, social and work environments. May facilitate non-therapeutic groups, address events/crises and perform coordination of care tasks; performs related duties as required.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Provides direct support services to include engaging clients in Independent Living Skills (ILS) activities, health and wellness promotion, personal assistance services, transportation services and crisis management.
  • Observes, interacts, assists, advises and directly facilitates support to clients throughout the therapeutic day.
  • Observes and assists in clients’ self-administration of medications; observes and assists in client’s self-administration of blood sugar tests; administers basic first aid.
  • Performs client counts hourly during the day and conducts hourly bed checks and grounds checks nightly.
  • Prepares documentation regarding clients in accordance with regulatory requirements and company policies and procedures.
  • Oversees kitchen operations, prepares menus, teaches and assists clients in preparing, cooking and serving meals and monitors cleanup activities.
  • Purchases, receives and stocks foodstuffs and supplies as required; monitors food refrigeration.
  • Performs office tasks such as work orders, event notifications, copying, faxing, logging mail and answering phones; receives and verifies logs, keys and cash.
  • Provides transportation to clients as directed.
  • Facilitates clients’ recreational activities.
  • May assist in coordination of care tasks under direct supervision.
  • Participates in clinical staff meetings, trainings and other activities as required.

Job Requirements

KNOWLEDGE, SKILLS AND ABILITIES REQUIREMENTS

  • Knowledge of the principles of cultural competency and adherence to its philosophy.

  • Knowledge of Lifewell’s Code of Ethics statement and adherence to its concepts.

  • Knowledge of behavioral health services delivery systems policies and procedures.

  • Knowledge of state behavioral health individual and agency licensure regulations and requirements.

  • Knowledge of state and local behavioral health resources.

  • Ability to work effectively with others and to communicate effectively, orally and in writing.

  • Ability to maintain confidentiality of information.

  • Ability to work under pressure and address events/crises which may develop unexpectedly.

  • Knowledge of food preparation and kitchen operations.

  • Knowledge of the principles of safety, health and sanitation.

  • Knowledge of computer software appropriate to the assignment.

  • Skill in operation of a passenger van.

 

EDUCATION/LICENSE/CERTIFICATION/EXPERIENCE REQUIREMENTS

  • A High School Diploma/GED and one year of work experience.
  • Some assignments require an Arizona DPS Class One Fingerprint Clearance Card.
  • Some assignments require a current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR).
  • Some assignments require Food Safety Certification.
  • Must be 21 years of age.
  • Background Check.
  • Minimum 1 year recovery from drugs and/or alcohol.
  • Minimum typing speed of 25 wpm.
  • Possession of, or the ability to obtain, the following:
    • CPR and First Aid certifications;
    • Evidence of freedom from infectious pulmonary tuberculosis; and
    • Physical Exam at hire
