AZ-Clinical Project Manager - Nursing Home

Job Description

Under general supervision of the  Director, Nursing Home (NH), the NH Clinical Project Manager (CPM) provides quality improvement (QI) support to a defined number of Arizona NHs throughout the state.  Quality improvement support consists of conducting on-site visits, teleconferences, Webinars and Learning and Action Network (LAN) meetings with HSAG participating NHs.  The CPM also helps NHs with improvement of Nursing Home Compare quality measures using the Quality Assurance & Performance Improvement (QAPI) model. The CPM requires knowledge specifically in the reduction of antipsychotic medications and other quality measures specified by the Centers for Medicare & Medicaid Services (CMS).



ESSENTIAL COMPETENCIES, DUTIES AND RESPONSIBILITIES

  • Serves as a primary HSAG contact with assigned NHs and communicates/consults with them on a regular basis to assist with their interventions related to established CMS goals, objectives, and measures, and to provide technical assistance and recommendations as needed.


  • Assesses current NHs’ processes, support systems, and barriers related to the CMS QAPI focus. Based upon assessment findings and in collaboration with the NH team, determines appropriate interventions and modifications, and shares these with the appropriate NH staff.


  • Able to communicate and work well on tasks with Arizona stakeholders to plan, facilitate, and execute interventions.



  • Utilizes clinical expertise, problem-solving techniques, flow diagrams, statistical process control charts, Advancing Excellence Campaign tools and resources, QAPI and other methods to demonstrate to NHs how to reengineer existing processes or design new ones to improve performance.


  • Possesses ability to discuss complex health care issues during face-to-face meetings, teleconferences, and through written correspondence with providers and stakeholders.


  • Remains current and knowledgeable about new QI activities, opportunities, tools, and techniques through training, education, and a variety of nationally recognized sources in the NH profession.


  • Routinely documents and reports project status, on-site visits, provider interventions, activities, and barriers/solutions, using appropriate HSAG and/or CMS reporting tools and databases.


  • Strong working knowledge of the publicly-reported Nursing Home Compare (QMs) on www.medicare.gov as related to CMS contract requirements.






Job Requirements

  • Candidate must be an experienced healthcare professional with a minimum of a Bachelor's degree in appropriate science or related discipline. Arizona NHA RN, LPN or Social Work license preferred.


  • Three or more years of healthcare experience in a hospital and/or nursing home.



OTHER QUALIFICATIONS

 

  • Quality management experience and knowledge of QAPI tools and techniques.


  • Clinical experience specific to the reduction of antipsychotic medication and other clinical quality measures required.


  • Experience specific to NH management is preferred.


  • Possess general understanding of the changing health care environment.


  • Proficient in Microsoft Office systems.


  • Proficient in oral and written communication and interpersonal skills.
