Job Description

A Home Health Care Agency located in the Greater New York City Area is seeking a Transplant Coordinator. 

Description:

  • Will be paged when a new case comes in & then will have to go to that hospital - they try to keep them in assigned territories - from Poughkeepsie to Montauk. 24 hour coverage - work 10 days in a 28 day cycle

Responsibilities include:
  • Assessments, observing and documenting vitals, assessing reflexes for signs of brain death, assisting in the facilitation of brain death declaration, observing and documenting apnea test results, verifying proper brain death documentation with hospital policy, reporting to clinical managers on patient status, relevant history, and plan of care, ordering and obtaining pertinent labs, consults and procedures (EKG, Chest X-ray, Echo, Cardiac Cath, Bronch, central line placement, A-line placement, dialysis, etc), monitoring I & Os, obtains medical and social history, ensuring proper antibiotic coverage of positive cultures, drawing serologies, interpreting serologic results to identify unsuitable donors/ suitable for some organs and tissue, sending out tissue typing, identifying high risk behaviors, approaching families for consent, counseling families, initiating  diligent search of LNOK, hospital development, confirming LNOK, updating clinical managers, tissue department and OPC of patient status, allocating organs, obtaining ME clearance, obtaining death certificate, arranging an OR time, arranging pick up time for surgeons, discussing and help monitoring of donor management with anesthesia, reviewing donor chart with recovery teams, arranging intra-operative biopsies,  label, package, and arrange for post-recovery tissue typing materials, package and arrange transportation for recovered organs locally and out of region, assisting with post-mortem, calling ME after completion of case, obtaining op notes from surgeons, notifying family, FSC, OPC, tissue department, and clinical manager of case completion and findings, attended staff meetings 
  • Review referrals from medical centers throughout the NY Region for potential organ/ tissue donors. 
  • Make on-site visits to evaluate and determine donor suitability; work collaboratively with facility medical teams to optimally manage organ viability through strict adherence to established brain-death/ terminal wean protocols. 
  • Play an active clinical role in management of donors; perform physical assessments; obtaining serology & tissue-typing specimens; serve as a consultant & educator to all hospital staff involved. 
  • Serve as a resource in maintaining optimal organ viability including diagnostic evaluation, treatment issues & procedures. 
  • Serve as liaison to families; ensure preparation for end of life decisions and provide grief counseling. 
  • Coordinate process of organ sharing through the national database after consent signing; collaborate with transplant centers/facility administrators throughout the United States, arrange air flights, contact O.R.'s on stand-by, funeral directors and others. 
  • Remain in O.R. during procurement; maintaining on-going clinical communications w/ multiple recipient transplant surgeons; coordinate adequate organ perfusion/medications w/ anesthesia, oversee perfusion, sterility & distribution, biopsies etc.; assist w/ post-mortem care; communicate w/ Medical Examiner & Funeral Directors as needed. 
  • Serve as an educator to hospitals and public outreach programs to maintain clinical & family/grief counseling competencies & to promote the benefits of organ donation. 


  • Travel: In this capacity, you will work in 12 hour pre-scheduled shifts. Travel to all NYC boroughs, Long Island and Westchester is required. Travel expense is provided. Additional reimbursement for tolls and parking is available. 

Job Requirements

  • NYS Licensed and currently registered as a Registered Professional Nurse.
  • Degree in Nursing or an Associate’s Degree in Nursing.
  • 3+ years experience in ICU/CCU
  • Must be flexible for travel
