Job Description

INFUSION Pharmacy Technician

Must be a certified Pharmacy Technician (CPHT). Maintains dispensing and compounding systems  including  Baxa EM1200 and Repeater Pump, repackages medication. Demonstrates advanced pharmacy technician skills and accepts additional responsibilities such as, chemotherapy preparation, and mixture of hyperalimentation fluids




  • Implements the Company Mission Statement.
  • Understand and operation and cleaning necessary for the Baxa EM1200 compounder and Repeater Pump
  • Participate in the fulfillment process and pharmacy support responsibilities including all aspects of IV compounding (see addendum).
  • Pack and ensure shipment of boxes to patients and facilities as needed; may require lifting boxes of varying weights
  • Communicate performance and quality standards expectations to fellow teammates (technicians and support staff)
  • Ensure appropriate quality controls are in place and communicate to pharmacy teams and facility partners in situations in which quality may have been compromised.
  • Consistent, regular, punctual attendance as scheduled is an essential function of this position.
  • Participates in Improvement of Operational Performance (IOP) activities.
  • Participates in traditional and e-learning programs.
  • Participate in the fulfillment process and pharmacy support responsibilities including all aspects of IV compounding.
  • Verify all information for patient from Intake for accuracy – contact patient or confirm that the pharmacist reviewed all intake information for accuracy. Compare to system information, checking DOB, address, phone numbers primary and alternative, next of kin, whether delivery can be left on porch or with neighbor if does not require signature, check for HIPAA approved contact other than patient, insurance information etc
  • Verify medical authorization has been obtained for all S-codes (Nursing codes and Per diems) and J-codes (NDC’s) within the Insurance Verification process.
  • Prepare IV admixtures for patients – Follow protocols: prepares clean room and hoods. Pulls appropriate medications and diluents and admixture supplies patient-prescription specific compounding. Obtain pharmacist check prior to passing through to the clean room.  If compounder or pooled products then have the pharmacists check all settings. Post compound: prepare for pharmacist check and counter sign per patient.
  • Pick and pack patient specific supplies – double check the pharmacist and vice versa – two signatures per order
  • Delivery Log utilization: Determines the route of delivery (Fed-x, USP, USPS or driver) with consideration for delivery date/times, needs/constraints: including IV mix schedule, traffic patterns and holidays, etc.  Verifies signature on receipt. Tracks expected and actual time of delivery on the Delivery log. Documents/confirms delivery on the Delivery log for all routes of delivery.
  • Cleaning process (daily, weekly, and monthly) with adherence to USP 797 including ante room, clean room, chemo room, and medication room.  Records cleaning on the appropriate Environmental cleaning log.
  • Records Temperature of the refrigerators, freezers, and clean room, incubator and pressure differentials on the appropriate Environmental Temperature log.
  • Monthly manual drug/medication inventory and reports the drug count, lot numbers/expiration dates, cost, AWP and ASP on spread sheets. Updates pricing monthly. Responsible for the Best price List and the Price comparison List for medications and supplies. Verification of computer software pricing and Vendor pricing with medispan verification also.
  • Quarterly manual supply inventory and reports supply count and cost on spread sheets.
  • Monitors for soon to expire and expired medications, remove expired medication from inventory and document on the Expired Medication Log.
  • Responsible for all ordering of supplies and drug/medications from vendors. Once shipment is received check shipment against Invoice.  Problem solves any discrepancies. Logs shipment inventory into computer system.
  • General cleaning of pharmacy facility including but not limited to vacuuming/sweeping of bathrooms, kitchen, and warehouse.
  • General ordering of office supplies through the corporate office. General office maintenance of office equipment (computers, copier, scanners, phones), office generator, fire extinguishers and environmental heating and cooling systems.
  • Electronic scanning and original document maintenance of patient clinical, compounding and financial charts.

High school diploma or general education degree (GED); Certified Pharmacy Tech (CPhT), one year of experience as a pharmacy technician. Chemo training strongly preferred. IV infusion experience required.



Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.  Ability to write routine reports and correspondence.  Ability to speak effectively before groups of customers or employees of organization.



Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.



Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.



To perform this job successfully, an individual should have knowledge of Word Processing software; Accounting software.



High school diploma or Equivalent required.

Certified Pharmacy Tech (CPhT) required.



This job has no supervisory responsibilities.
