Job located in Arlington, Texas

Texas Health Arlington Memorial, a 369-bed MAGNET-designated hospital, is in search for a Diabetes Educator for its Inpatient Diabetes Management department. If you are looking to impact nursing education, community development, and patient care management, this may be a great fit for you!

Job Requirements

Qualifications

  • Graduate of a NLN (National League of Nursing) or CCNE (Commission on Collegiate Nursing Education) accredited nursing programs. For RNs hired after Jan 2014, completion of BSN within 3 years of  RN start date as defined in offer letter. Effective Dec 2015, completion of BSN within 2 years of RN start date as defined in offer letter
  • Registered Nurse (RN) license to practice in Texas or Compact State
  • Current Certified Diabetes Educator (CDE) or obtain CDE within 6 months of meeting the experiential requirements for application for certification. If non-CDE, must meet annual continuing education requirements by the American Diabetes Association for meeting National Standards for Diabetes Self- Management Education and Support
  • 3 years clinical experience
 
Responsibilities include, but are not limited to:
  • Provide consultations, demonstrations, or instructions related to staff development for improved patient education. Provides consultation to professional colleagues, patients, and families regarding effective self-management skills, treatment plans, etc.
  • Promote/facilitate the advancement of diabetes information for staff. Identifies learning opportunities specific to the individual or the group needs of patients or staff; plans and provides formal and informal education to meet those needs; utilizes appropriate educational resources that are culturally sensitive and meets the learning needs of patients/families. Education programs reflect current, evidence-based information and pertinent research findings as indicated.
  • Actively participate in interdisciplinary patient care conferences contributing to the treatment plans of patients as indicated
  • Lead multi-disciplinary teams based on Wagner's Model for standard screening and treatment protocols.
  • Develop partnerships with local physicians and healthcare agencies within and around the county to facilitate patient access to services and resources needed to support clinical care for management of chronic conditions, etc.
  • Document services rendered within the medical record reflecting thorough and timely record keeping same day as service provided.
  • Monitor patient evaluation and treatment plans for compliance to national guidelines and best practices; shares results through the Performance Improvement Committee and interdisciplinary team rounds as indicated.
  • Create and maintain databases for tracking patients enrolled in self-management programs; identifies, monitors and documents key outcome and compliance data for diabetic patients following discharge from the ED in concert with other colleagues.
  • Assist with Advisory Board and ADA reports. 
  • Provide feedback to community providers as needed.
  • Use advanced interviewing and assessment techniques to obtain patient biophysical, psychosocial and self-care education and discharge needs of those diabetic patients identified at risk and/or underserved using the ED for chronic management of their diabetes. In doing so, works closely with ED Navigator and Transitions Team members.
  • Integrate appropriate assessment of the patient's readiness to learn, barriers to learning (illiteracy, language, need for communication aids in the geriatric population, etc.), preferred learning methods, cultural influences, age appropriateness, etc. into the individualized plan for the patient. Educate and assist diabetes patients through appropriate evaluation and teaching methods.
  • Identify specific, individualized diabetes self-management goals; plans and implements education and behavioral interventions directed toward helping the individual achieve the identified goals; develops teaching plans reflecting acceptable standards, monitors for compliance and attainment of goals intervening as needed, and, maintains accurate documentation records related to all education encounters.
  • Identify methods for developing new tools to enhance instruction methods both in the ED and in the community.
  • Utilize effective interpersonal and communication skills to achieve desirable outcomes and responses.
  • Develop relationships with varied community agencies, healthcare resources and physician providers to support the varied needs of the identified diabetic patient population.
  • Actively engage in community events affiliated with THR as needed to facilitate visibility and health promotion related to the management of diabetes.
