Comforcare is a provider of top quality private-pay non-medical in-home care for elderly people.  We are looking for a team member who is passionate about doing excellent work, and would enjoy being an important part of the success of our company.

Job Description: This individual will oversee the caregiving staff and will be responsible for client and employee satisfaction.

The Case Manager is responsible for all components of Comforcare’s care delivery.  This begins with assessing new clients, creating their care plan, and determining the best way to train aides for their first shift with the client.  The manager will introduce new caregivers to clients, and follow up with clients and caregivers to ensure things are going well.  The manager will conduct supervisory visits of aides and identify training needs.

The Case Manager will also assist with meeting with families considering hiring Comforcare, which involves answering questions, addressing concerns, and communicating what is unique about Comforcare.

The Case Manager will be cross-trained in scheduling, and answer after-hours office phone calls one week per month.  This person must be motivated to learn all aspects of the business, and to contribute to the overall success of it.  Although this position is part time, Comforcare’s staff is providing care 24/7 so the management staff  are often called upon after business hours.

Comforcare’s non-medical care duties include: personal care (bathing, toileting, dressing), homemaking (cooking, cleaning, etc.), medication reminding, transfers for those unable to walk, transportation, and companionship. Client needs range from someone needing help just a few hours per week, through someone who is bed bound and on hospice care.

A social worker with experience with the geriatric population would be an ideal fit for this position.  Excellent verbal, written, and organizational skills.  Excellent follow through with details.  Confident giving guidance to aides.

Additional Qualifications: Experience in: assessing individual’s situations and identifying needs. Ability to work independently, self-motivated, excellent and thorough with detail (and paperwork), passionate about quality of care, confident in making recommendations to clients, family members, or other care providers, confident in promoting Comforcare’s services, good with directions and finding new places, flexible to a schedule that is unpredictable , solid computer skills (and a home computer and internet service).

Clients are located primarily in Denver, Littleon, and Aurora, and the office is in Aurora.

Willing to take full ownership of responsibilities (despite part time schedule), which often means following up on details that can’t get taken care of during regular work hours.  Willing to be reachable during non-work hours.

Hours:  Could be full or part time.   Approximately 80% during business hours, and 20% after business hours (early morning, evening, weekend).  Much of the time will be spent in the field with clients.
