Here is what you'll get to do:

The Health Partner I will support CCHA initiatives and the Primary Care Medical Provider’s efforts in providing a medical home for Medicaid members. The Health Partner will work to maximize patient use of existing health services and assist and guide patients through the health system.  They will assess members to ensure that patients are able to access and keep timely and appropriate healthcare services, participate in recommended care plans or follow-up, and receive appropriate health information about their diagnosis and treatment plan.

Job Requirements

Here is what you'll need to succeed:
  • Minimum of High School Diploma or GED.
  • Previous experience working in a health care setting with Medicaid preferred.
  • Previous experience with inbound or outbound calls in a healthcare setting.
  • Working knowledge of basic medical terminology preferred.
  • Bilingual (English/Spanish) highly preferred.
  • Knowledge of basic care coordination and experience in working with local community resources for both medical and non-medical services.
  • Good verbal and written communication skills and the ability to convey information clearly to others.
  • Basic computer and internet skills required for routine data collection, data entry and reporting: experience with Microsoft Word and Excel spreadsheets.
  • Ability to identify and resolve problems in a timely manner; and to work well in group and independent problem solving situations.
  • Excellent time management skills and ability to complete tasks on time; ability to multi-task.
