Case Manager

Job Description

We are looking for a Case Manager to be part of a creative interdisciplinary team to conduct home visits, assess consumer needs, develop and manage care plans and act as an advocate for consumers. The Case Manager balances his/her schedule of visiting consumers in their homes and working in the office taking phone calls from consumers, coordinating services and participating in team meetings.

Job Requirements

  • Bachelor's degree required with social service experience.
  • Truly comfortable using a computer.
  • Sensitivity to the needs of seniors and individuals with disabilities.
  • Valid driver's license and reliable vehicle.
  • Ability to actively participate in self-directed team environment with excellent communication and documentation skills.
  • Bilingual a plus.
