Catastrophic Case Manager

Job Description


The Catastrophic Case Manager (CCRNCM) is responsible for providing care management services to members of the organization's Medicare and Medicaid health plans of California.

 

Key responsibilities include:
  • Intense complex care management of members meeting the criteria of catastrophic needs. 
  • These will include those members identified with acute care hospitalization(s)
  • More than one ER visit in a quarter, all cause readmissions to acute hospitals, transplant work-up/wait list/follow up, skilled nursing facility stays greater than 14 days, high utilization of outpatient/home health services.  


Job Requirements

  1. CA RN
  2. ACLS
  3. 3+ years clinical experience in case management
  4. Spanish speaking preferred
  5. Bachelors in nursing or related preferred
