Job Description

Full-time, Sunday - Thursday, 8:00 am – 5:00 pm,

As a Patient Service Specialist you will provide specialized support for clinical staff and managers that facilitates our work with patients and families.   You will regularly communicate with doctors, managers and staff to clarify, coordinate or obtain additional information needed for effective patient care. You will assure all forms are completed, properly identified, signed and all required information is entered in the electronic medical record (EMR). 


  • Reviews chart for required documents and key elements upon admission, forwarding required documents to physician.

  • Communicates referral information to  vendors , sends payment responsibility forms and physician orders as needed, and tracks to ensure results or notes are obtained afterward.

  • Arranges and tracks pending laboratory tests, forwards results to staff, and ensures final results are obtained if indicated.

  • Monitors incoming and outgoing electronic fax program for faxes. Forwards to appropriate staff, assists in resolving issues, saves patient documents to EMR, provides support to clinical staff when needed.

  • Enters orders for durable medical equipment

  • Prepares records for release, ensuring accuracy of signed release forms and documents. Information released according to state and federal laws. 

  • Ensures billable sections of the EMR are accurate for billing to occur.

  • Communicates throughout the day with clinical staff and managers regarding moves, respites, hospitalizations, discharges, deaths and any other clinical concerns/issues.

  • Reviews chart for completeness and accuracy of key elements on discharge.

Job Requirements

  • High School diploma or GED required.  

  • Health Information Management or Medical Office Management education preferred.  

  • Formal training and/or work experience in a health care setting required. 

  • Knowledge of medical terminology required.

  • Two years experience in various aspects of medical office practices required. 

  • Typing, filing, multi-line telephone, data entry, report processing and facsimile transmission skills required. 

  • Knowledge of basic HIPAA rules required.

  • Excellent computer literacy required.

  • Bi-lingual skills and fluency in English/Spanish desirable.
