Job Description

Under the supervision of the Manager of the CCF and the Medical Director of Asthma Education (CSRHC), and through collaborative efforts, the Social Worker II plans, develops, coordinates and implements a structured intervention for children with asthma and their families.  The Asthma Counselor will provide individual and group training sessions for both children and families.  Subsequent individual interventions will be tailored to the individual family.  The Asthma Counselor will maintain program data and monitor for quality.

 A. Performs comprehensive assessments which examine patient/family current problem (s) as well as relevant biopsychosocial and environmental factors.  Communicates relevant aspects of the assessment to patient, family, and members of the health care team.  Collaboratively formulates appropriate and realistic plan for individualized intervention, working with other members of the health care team and available community based organizations. B. Provides group and individual training sessions for both children and their parents, using an approved curriculum.  In addition, individual interventions will be tailored to the family to include home visits if necessary.
C. Maintains program records and assures the quality and completeness of the evaluation data.  
D. Participates in ongoing education; assures that new information is put into practice.
E. Uses supervision and consultation for professional growth and to provide more effective services to patients and families.
F. Serves as a consultant and educator to the interdisciplinary team regarding the psychosocial components of illness, disability, and hospitalization.
G. The knowledge and skills necessary to perform the position requirements are demonstrated through the successful completion of the individual competencies established for the position, to include age specific and other special needs of patients or customers, served by the department.
H. Demonstrates competence to perform assigned patient care responsibilities in a manner that meets the population-specific and developmental needs of patients served by the department.

Job Requirements

Qualifications:

A. EDUCATION:

1. Master of Social Work degree from an accredited college or university is required.     

B. EXPERIENCE:
1. Two (2) years post masters experience involving direct patient contact and family interaction in a hospital or health care facility is preferred.

C. LICENSURE/CERTIFICATION:

1. Must possess a valid license (LMSW) to practice social work in the state of Texas.  
2. Certification by the National Asthma Educator Certification Board (NAECB) is preferred. If associate does not already possess this certification, they will be expected to obtain it within one year of hire.
