Family Service Coordinator (Social Services)

We are currently seeking a Family Service Coordinator to provide exceptional outpatient therapeutic intervention services. This position reports to the Rehab Specialist and provides direct services to clients by providing services including intake assessments, individual / family / group therapy and crisis interventions.

The ideal candidate must possess the following:
  • Bachelors’ degree from an accredited University or College specializing in psychology, sociology, social work, human services, and/or anthropology
  • At least 1 year of experience in a mental health setting
  • Bilingual preferred

Duties and Responsibilities:
  • FSC are required to assess the needs of the client throughout their time in the program
  • Provides services on a weekly basis, either in the office or in the community
  • Completes all notes (direct and in-direct) on our EMR (electronic medical record software) daily
  • Complete treatment plan updates on our EMR for each client that details the client's progress towards goals, response to services and goals that were the focus of the services, the client's response to the services and a follow-up plan of action
  • Attends weekly team meetings and training as scheduled and required
  • Participates in bi-weekly supervision with the Rehabilitation Specialist, during which client progress, setbacks, and difficulties are discussed
  • Collaborates with the client's assigned mental health therapists monthly and documents the type of collaboration (i.e. phone call, attending a meeting, etc.)
  • Coordinates and facilitates monthly groups for Therapeutic Living for Families clients. 
  • Maintaining 70% direct service requirement
  • Other duties as assigned
Additional Requirements:
  • Daily and/or weekly home visits to clients’ homes
  • Use of own vehicle for visits to clients’ homes
  • Valid driver’s license
  • Must have experience working with children and adults
