Job Description Summary:

The Private Duty RN Case Manager is a Registered Nurse who directs and supervises all personnel involved in the client care within a specific geographic area. They are responsible for the total plan of care and services of clients and the allocation of agency resources. The RN Case Manager will manage private duty clients, is responsible for opening new private duty cases and performs supervisory visits as directed by the Clinical Manager. Participation in team conference, community events, EMR management and excellent customer service delivery are key requirements of this role. Additionally, the Private Duty RN Case Manager is tasked with helping to monitor renewable requirements for all field staff.

Essential Job Functions/Responsibilities:

A.Make initial client evaluation and assessment visits. Consult with client, family members and office staff in establishing, reviewing and updating the plan of care and services as necessary.
B.May review new case referrals. Reviews available client information related to the case, including need for disciplines that may be required to meet the client’s home care and service needs. Communicates client information to the Clinical Manager and Client Services Manager
C.Help evaluate the need for other services outside of the regular private duty business line, i.e. skilled nursing, skilled nursing visits.
D.Consults with Clinical Manager and Director of Operations with any issues or problems concerning the admission of new clients regarding client care and agency services and works to ensure appropriate coordination of services as needed.
E.Reports all matters requiring further action to Clinical Manager and/or Director of Operations.
F.Assists Clinical Manager in the supervision of home health aides, certified nurse’s aides, nurses and/or other ancillary service personnel ( if applicable ) and:
a.Is knowledgeable of the client and their needs/services.
b.May receive reports from other employees caring for the client.
c.Supervise home health aides and companions assigned to render client care or services within a specific area as it relates to the client’s established plan of care.
G.Review client treatments and notifies appropriate team members of any changes that may pertain to them and ensure that the Clinical Manager is informed at all times of the client’s current status.
H.Participates in the nurse on-call system of the agency as appropriate and assigned.
I.Attends nursing service meetings and combined care provider meetings for care coordination purposes or other reasons as scheduled by the agency and/or management staff.
J.Participates in staff development activities as assigned and according to agency policy and procedure.
K.Strives to continually improve professional nursing knowledge through agency provided educational activities, on-line opportunities for learning, attendance at workshops, conferences, professional organization meetings and/or individual research and readings.
L.Participates in the evaluation of the agency nursing program to ensure that care and services provided are adequate, appropriate, effective, and efficient and provides input to The Clinical Manager, the Director of Operations or appropriate corporate leadership if any opportunities for improvement are identified and/or resources identified are needed to improve client care and services.
M.Initiates and conducts regularly scheduled Care Team Conferences for their clients and ensures documentation of meeting minutes is reflected in the client’s record and communicated to the team per the Care Planning and Coordination Policy.
N.Conducts phone follow-up with clients per agency protocols and/or as the client condition may require.
O.Completes assessments, re-assessments and client visits and provides related supervision in a timely manner.
P.Determines the need for additional supervisory assessments and provides the updated documentation in the client’s home in a timely manner.
Q.Reviews medical records for assigned case load and assures that the client’s care and service record clearly outlines the reason for admission for care and services, the status of the patient throughout service, the delivery of care and services according to the plan of care and physician’s orders (if applicable) and records the patients responses to care and services provided, including satisfaction with care.
R.Reviews records to assure that all records are in compliance with agency policies and procedures. Ensures that all items are in place according to agency practices to ensure appropriate billing occurs.
S.Follows all privacy policies of CareSouth and maintains the confidentiality of protected healthcare information (PHI) at all times.

Job Requirements

Position Qualifications:

1.Licensed Registered Nurse in the state served by the agency.
2.Possesses and maintains current CPR certification.
3.A minimum of three (3) years of experience as a professional nurse is preferred, along with at least one (1) year of experience in the field of private duty, home, or community health.
4.Completion of a Baccalaureate Degree program approved by the National League for Nursing for Public Health preparation is preferred.
5.Must have reliable transportation, a valid driver’s license and current automobile liability insurance.
