We have an exciting opportunity for a bilingual Cantonese Health Coach, RN. As a Health Coach, you will be responsible for providing care coordination and care management services for Brown & Toland members. Activities include: assessment, identification, planning, implementation of an individualized plan of care, and evaluation of outcomes. Collaborates with internal and external stakeholders in coordinating a plan of care to achieve optimal quality healthcare outcomes for Brown and Toland members.

  1. Provides health coaching activities across the continuum of care in order to facilitate and promote high quality, cost effective outcomes, focused on the whole patient orientation and self management decision support and aims to minimize any fragmentation of health care delivery.
  2. Acts as the liaison between the member, the member’s support network, treating physician(s), and ancillary providers utilizing a multi-disciplinary approach to assess, identify, plan, health goal setting and measure optimal clinical outcomes.
  3.  Employs behavior change/motivational interviewing skills to assess readiness, health goal setting short and/or long term needs, engage member’s plans for change following standard policy and procedures, clinical guidelines and national evidenced based criteria.  
  4. Works collaboratively with the Care Management team, Health Care Analytics, Performance Improvement departments, Health Plans and Brown & Toland physicians and their offices to identify individuals who require care management wellness and prevention services.
  5.  Masters the use of Brown & Toland e-tools to identify and target needs of risk populations.
  6. Develops patient outreach activities such as mailings, reports as well as physicians level quality performance reports.
  7. Support development, implementation and evaluation of any Brown & Toland programs and activities to improve optimal health outcomes.  
  8. Acts as patient advocate responding to and working to resolve patient concerns or barriers.
  9. Assist in the development, implementation and evaluation of standards of care, policies, procedures, medical review criteria and protocols to improve care management throughout the delivery system and across the care continuum.  
  10. Serve as a resource to members and B&T physicians. Maintains up to date information on community resources, health coaching literature or recommended screenings and community based programs.  
  11. Communicate timely with primary care providers, specialists and any community affiliates to ensure continuum of care. 

Job Requirements

  • Three years min., years clinical setting experience as an RN health coach required (or equivalent experience)
  • Managed care experience preferred, preferably in an HMO or IPA setting
  • California licensed RN or APRN preferred
  • Knowledge of Manage Care concepts, Health promotion strategies, Education background, preferred
  • Excellent verbal and written communication skills
  • Proficiency in Windows-based database applications
  • Strong organizational skills with the ability to prioritize
  • Strong customer service skills
  • Must be able to work independently as well as work well with teams
  • Ability to maintain a high volume workload
  • Ability to effectively establish rapport with members via telephone
  • Must be able to problem solve and manage conflict
