The Intensive Case Management (ICM) Case Manager is responsible for the facilitation of Child & Family Teams for children with high needs.
ESSENTIAL DUTIES & RESPONSIBILITIES:
•Develops and monitors Strengths and Culture Discovery summaries, treatment plans, and crisis plans with clients and participating agencies.
•Assists clients in securing a variety of social services.
•Maintains client records in accordance with regulatory, contractual and agency requirements.
•Provides complete documentation and reporting of all client services in accordance with regulatory, contractual and agency requirements.
•Meets with Child & Family Teams to review utilization of all services, both formal and informal.
•Monitors client progress towards treatment goals.
•Attends and participates in team meetings.
•Participates in ongoing program planning and development.
•Must be able to safely and effectively monitor consumer activities, with or without a reasonable accommodation.
•Performs other duties as assigned, which are consistent with the general responsibilities of the position.
