Clinical Nurse Specialist, STEMI Program Coordinator

Job Description

    JOB SUMMARY

    The STEMI Coordinator is responsible for facilitating the coordination of all services/systems required for the care of the STEMI patient. The STEMI Coordinator functions in support of and collaboration with other health care team members by serving as an advanced clinical practitioner, consultant, educator, researcher and change agent. The position supports the goals of St. Joseph and St. Mary’s Medical Center in the advancement of the provision of quality STEMI care.

    The STEMI Coordinator adheres to the standards set by state, federal, and private regulatory agencies such as but not limited to the Joint Commission on Accreditation of Healthcare Organization (JCAHO), The Conditions of Participation by The Centers for Medicare and Medicaid Services (CMS), the American Heart Association’s “Get with the Guidelines', the Missouri Division of Health and Senior Services Time Critical Diagnosis (DHSS) Certification and the Society of Cardiovascular Patient Care Accreditation. The position operates within the scope of practice as outlined by the Missouri State Board of Nursing.

    PRIMARY RESPONSIBILITIES

    • Serves as a clinical resource/consultant for nursing and ancillary staff related to STEMI care. Evaluates all aspects of STEMI care as appropriate for efficiency, quality, effectiveness of standards, and clinical pathways and protocols.
    • Demonstrates advanced cognitive, affective and psychomotor capabilities in the nursing assessment, diagnosis, intervention and evaluation of human responses to actual or potential health problems.
    • Participates in the development and implementation of patient/family STEMI education and discharge/transfer planning.
    • Participates in development/implementation of STEMI orientation and continuing education programs for staff.
    • Reviews patient’s medical records for particular aspects of care and/or problems that adversely affect care. Facilitates and role models appropriate STEMI care. Provides leadership in performance improvement activities for STEMI care. Coordinates to revise STEMI patient care protocols and systems issues when necessary.
    • Assumes responsibility and accountability for the STEMI - Get with the Guidelines (GWTG) as an integral component of the STEMI CQI process. Facilitates GWTG ACTION into the ACTION registry and where appropriate oversees data collection.
    • Participates in the implementation to meet any federal, state, or private guidelines and regulations related to STEMI care.
    • Displays value for professional nursing practice including the utilization of current standards, the nursing process and patient advocacy.
    • Demonstrates decision-making and problem-solving abilities that provide effective prioritization of patient needs.
    • Utilizes current research findings in development of nursing practice interventions, policies and procedures, protocols and outcomes.
    • Seeks to improve patient and family outcomes through the application of educational concepts and skills.
    • Contributes to scientific knowledge base by utilizing, facilitating, conducting, presenting, and publishing scholarly works and research findings.
    • Assures ethical and legal practices in the conduct of research.
    • Utilizes knowledge base related to various development stages of patients and applies this knowledge in the planning and implementation of care for individuals, groups and populations.
    • Supports the concepts of shared leadership and the implementation of professional practice.
    • Assumes responsibility for own personal and professional development seeking opportunities for learning.
    • Serves as active participant on Clinical Support Team and Nursing Leadership Team.
    • Facilitates the development, implementation, compliance and evaluation of standards of patient care, practice and policies and procedures for the STEMI program.
    • Maintains current knowledge of STEMI care and future trends through journals and professional association contact.
    • Coordinates and facilitates the activities related to development, implementation and analysis of clinical pathways with designated physician leaders.
    • Assists in development and implementation of programs and services related to the STEMI program.
    • Evaluates the flow of STEMI patients, evaluates care, and resolves problems related to the care delivery process.
    • Collaborates with the STEMI Medical Director and Chest Pain Committee to conduct audits of system problems.
    • Assists with marketing the STEMI program to internal and external audiences.
    • Serves as a resource to both internal and external patient care givers by accepting consultation requests from medical center departments, administrators, EMS providers, and regional referral sources on issues relating to STEMI patient care, STEMI care delivery systems, quality improvement and marketing.
    • Works with EMS and ER staff to optimize the handoff of the Cardiac patients. Coordinates a regional system of care with EMS and referring facilities.
    • Identifies resources needed to meet the goals and functions of the STEMI program.
    • Provides input to develop annual budget for the STEMI program.
    • Participates in the multidisciplinary oversight of the continuum of care for STEMI patients, fosters the development of an integrated STEMI delivery system by identifying patient problems, nursing intervention, physician process, intermediate and final outcomes, and the time interval for treatment and outcomes throughout the patient’s hospitalization.
    • Participates in institutional committees/task forces which influence or determine policies affecting nursing practice related to STEMI care.
    • Performs other duties as assigned.

    Job Requirements

    MINIMUM REQUIREMENTS

    EDUCATION/TRAINING

    • Graduate Degree: MSN

    EXPERIENCE AND SKILLS (with minimum requirements indicated as appropriate)

    • 2 years recent clinical experience in related clinical area.
    • Demonstrated experience in managing complex projects. Detail oriented.
    • Excellent written and verbal communication skills intra-departmentally and inter-departmentally.
    • Ability to work effectively with professionals of diverse disciplines.
    • Ability to deal with individuals in stressful situations using counseling techniques and/or crisis management skills.
    • Effective change agent.
    • Analytical skills necessary to gather data, determine conformity based on predetermined criteria, identify problems, and design recommendations to advance quality STEMI care.
    • Good working knowledge of management theory and practices, principles of total quality management, group process, coaching, facilitating, supporting and communicating skills, research methodology, statistical tools and variance analysis, health laws and regulations.
    • Good working knowledge of system operations.
    • Good working knowledge of Medical Staff organization and functions.
    • Good working knowledge of clinical pathway development.

    CERTIFICATES AND/OR LICENSES

    • Current certificate, if available, from nationally recognized accrediting body
    • Current RN license in the state of Missouri
    • BCLS
    • Certification as APN in the state of Missouri
