Job Description

Evolution Health is seeking a candidate for our Healthcare Scheduler opportunity in Dallas, Texas.  The scheduler will direct and confirm patient appointments in an assigned territory for the Physicians, nurse practitioners, podiatrists and LCSW’s. The successful candidate will be responsible for building efficient assignment schedules using mapping tools to efficiently route providers in the best order and with the least amount of drive time between patient visits and the least traveled miles.  If you are motivated, enthusiastic, and looking for an opportunity to help service our patients, this is the opportunity for you.

Essential Duties and Responsibilities:

  • Responsible for proactively calling patients to schedule routine follow up visits
  • Responsible for mapping provider’s schedules and routing schedules most efficiently with the least amount of drive time.
  • Responsible for daily reporting on visits completed by providers, daily communication with providers while out in the field, regarding any missed visits, adding patients, calling patients if they don’t answer, and help locate patient home address if Provider unable to locate.
  • Responsible for scheduling new patients in designated areas based on location of the patient and availability.
  • Responsible for taking incoming calls from patients, providers, HHA’s and DME companies requesting visits. Making outbound calls to patients to confirm visits.
  • Adhere to all company policies and procedures.
  • Adherence to and compliance with information systems security is everyone’s responsibility. It is the responsibility of every computer user to: Know and follow Information Systems security policies and procedures. Attend Information Systems security training, when offered.  Report information systems security problems.

Non-Essential Duties and Responsibilities:

  • Responsible for answering questions other departments might have related to scheduling and directing patients to the correct office staff.
  • Responsible for providing coverage when a team member is not at work. Requires gathering daily report, taking phone calls, and completing schedules for the next day.
  • Perform other duties as assigned.

Minimum Qualifications:


  • HS Diploma or equivalent,


  • Healthcare experience (preferred), Medical scheduling, customer service or dispatch experience (preferred)

Knowledge and Skills:

  • Effective oral, written and verbal communication skills
  • Ability to use / operate standard office equipment and tools, outlook, excel, word, EMR system including visit note, VOIP Phone system, knowledge of insurance plans, ability to operate eligibility verification
