The Service Coordinator assists the clinical staff in identifying and accessing appropriate care and other social services for members.

Job Requirements


  1. Accepts phone calls from members, providers or other service agencies that are requesting additional information on the available clinical services and programs
  2. Triages calls and assists in any immediate crisis situations
  3. Verifies member eligibility for services through state and county information systems               
  4. Enters relevant call notes and information in FlexCare
  5. Maintains logs and other data bases regarding utilization management and care coordination activities
  6. Coordinates with clinical staff in accommodating members’ needs including transitions of care and access to timely appointments.
  7. Coordinates with other agencies and staff as needed
  8. Consistently provides members, providers and internal staff with exceptional customer service
  9. Provides resources to members
  10. Maintains schedule that allows for appropriate coverage
  11. Other duties as assigned
  12. Attends departmental and other meetings as necessary


Bachelor’s Degree in Human Services (Social Work, Psychology, and/or Sociology) preferred and/or 2-3 years experience in BH or other social services environment required. Word, Excel and PowerPoint strongly preferred.
