Job Description

As a critical member of the marketing and admission team, this position works closely with hospital discharge planners, physicians, social workers, case managers, management to generate referrals and ensures a smooth discharge process for patients transferring to Carondelet Manor.

Key Responsibility Areas


Works with referral sources (hospitals, physicians, social workers and case managers) to identify prospective residents

  • Obtains necessary information and documentation, review patient charts and plans of care, including medication lists, to ensure clinical appropriateness for facility and to determine if the patient meets Medicare guidelines.
  • Makes decisions regarding pre-admisssion assessments and interprets potential patient’s medical condition and past medical history while obtaining appropriate documentation.
  • Completes on-site hospital, facility and home visits to gather information regarding potential admissions.
  • Communicates with hospital staff in establishing a coordinated plan of care for patients who are admitted or awaiting admission.
  • Coordinates the completion of the admission process and ensures that appropriate resources are obtained for a successful transfer, providing appropriate information to facilitate continuity of care.
  • Collaborates with the attending physician to effectively manage the hospitalized patient and ensure that transfers are expedited as soon as possible.
  • Consults and collaborates with physicians, hospital staff and other professionals to provide clinical expertise, as appropriate.
  • Acts as liaison between hospital, physicians, nursing homes, other medical providers and Carondelet Manor to resolve problems, provide information and maintain positive relationships.
  • Identifies potential payer sources, establishes primary payers, document demographic/clinical/payer information and determine coverage availability for third party non-government payers.
  • Maintains current contact information on existing and new referral sources and prospective residents and regularly reviews and follows up.
  • Performs other duties as assigned


  • Develops and maintains a working knowledge of all services and resources provided by Carondelet Manor and available in the community.
  • Participates in and assists with the marketing activities and events as required.
  • Provide on-going communication with target referral sources including appropriate personnel at referring facilities, as well as, referring physicians.
  • Markets and promotes facility in order to maintain acceptable census and facility profitability.
  • Develops and maintains referral sources, educate others about the company, increasing visibility and reputation in the medical community, and identify market needs and trends.
  • Makes regular rounds (on-site and telephonic) of referral sources and maintains continuous communication with them and actively markets Carondelet Manor services.


  • Collects data regarding inquiries/referrals, pre-admission evaluations/outcomes and enrollment for regular monthly reports to Administration.
  • Participates in regular admissions meetings
  • Attends management meetings and required in-services.
  • Contributes to the admission process and protocol development, problems-solving and productivity enhancement in collaboration with medical and nursing leadership.

Job Requirements

Education, Experience and Professional licensure

• Education – LPN/RN

• Licensed State Registered Nurse  or Licensed Practical Nurse current in the state of Missouri

Knowledge, Skills and Abilities

• Knowledge- Medical case management, long term care and geriatric nursing

• Skills-- Strong medical assessment skills.

• Ability—Make sound judgments in evaluating and processing medical information. To handle medical case management with confidentiality
