Rehabilitation Case Manager
POSITION SUMMARY RESPONSIBILITIES
Responsible for coordinating the plan of care with the interdisciplinary treatment team for patients across the healthcare continuum. Coordinates the delivery of services to patients and families to facilitate and coordinate the discharge planning process for post-discharge needs. Serves as the patient and family advocate to coordinate and direct effective communication.
Master’s degree in Social Work from a school accredited by the Council on Social Work Education is required OR equivalent Master’s degree in Education/Psychology is required. Two years of professional experience in Rehabilitation is preferred. Bilingual in Spanish/English is preferred.
If applicable, current state licensure from the Texas Department of Human Services is required.
