The Case Manager will assume primary responsibility of cases for documentation of appropriate medical necessity for the inpatient or observation status. Communicates with physician, ancillary staff, and payers and assists in arranging transfers or alternate placement when indicated. Assures proper level of care assignment. Facilitates with Case Managers and other allied health professionals to promote and ensure that quality care is provided and outcomes are achieved in a timely and cost-effective manner. Utilizes a complete and systematic methodology/process to assess, plan, implement, evaluate and provide patient care coordination for patients from pre-hospital through post-hospital aftercare. Assists in the development of physician profiles to identify over/under utilization patterns. Promotes appropriate post-hospital utilization of resources, adequate education of patient and hospital staff, and decreased incidence of re-admissions. Supports hospital goals and objectives in compliance with regulatory guidelines. The Case Manager will work in collaboration with all healthcare disciplines to assure a continuum of appropriate and timely services.

Associate's Degree in Nursing, BSN strongly preferred.

Licensed RN in the State of Texas


Experience in one or more areas (multiple experience preferred): discharge planning, social services, utilization  management and/or case management,  Candidate must:
Demonstrate ability to interpret a variety of handwritten clinical data
Have concern for detail while being able to work quickly, accurately and with minimal supervision
Maintain a pleasant, tactful manner when working with people in stressful situations while solving problems/conflict
Be familiar with Medicare/Medicaid laws as well as requirements related to levels of care and financial assistance
Be skillful in dealing with physicians to resolve UM/DC planning issues
Demonstrate an understanding of medical record chart analysis
Experience with Interqual and M&R criteria for verifying hospital admission and continued medical necessity
Have a working knowledge of community resources and services
Act as a liaison between physicians, staff, patients and families
Language Skill:

Ability to read and communicate effectively in English.

Additional languages preferred.

Basic computer knowledge.

Ability to work effectively with computer based charting and other clinical software programs
Adaptability to change, organizational skills    
Ability to read and communicate effectively in English
