MDS Coordinator – RN Registered Nurse (Healthcare)


Job Responsibilities


As an MDS Coordinator with an RN background, you will ensure the accuracy of patient charts and medical data, schedule MDS assessments, and communicate the current schedule and any changes in procedures to the appropriate personnel or department.


Your specific duties in this role will include:


•Identifying and participating in process improvement initiatives that improve the customer experience, enhance work flow, and/or improve the work environment

•Participating in daily PPS meetings, weekly Medicare meetings, and month end meetings to ensure that federal billing requirements are met

•Assisting with the coordination and participating with the Interdisciplinary team in the completion and review of the Resident Assessment Instrument (MDS, CAA’s, and Care Plan)

•Notifying all Interdisciplinary team members of the MDS Assessment schedule for all payer sources

•Collecting information to complete the MDS using the medical record, bedside assessment, and staff, resident, and/or family interviews

•Ensuring that all Interdisciplinary Team members complete, date, and sign all assessments per Federal Regulations

•Verifying the availability of all MDS documents required either by Federal and/or State Guidelines or Organizational policy

•Ensuring that MDS assessments are transmitted in a timely manner

•Completing Care Area Assessments (CAA’s) in a timely manner per Federal Regulations

•Using information from the MDS Assessment and CAA’s to develop an Interdisciplinary Plan of Care

•Reviewing Plan of Care at least quarterly and with each Comprehensive Assessment to ensure that all changes are included and updated

•Leading or participating in Interdisciplinary Care Plan meetings

•Providing ongoing education of the RAI process to all interdisciplinary team members, direct and indirect caregivers, the business office, etc.

•Completing other special projects and duties as assigned

Job Requirements


As an MDS Coordinator, you must be a dedicated and professional team player with a flexible and adaptable outlook and the ability to self-manage. You must also be highly organized and detail-oriented with strong analytical, problem-solving, and prioritization skills as well as the ability to simultaneously manage multiple projects while maintaining a high level of quality. It is also important that you display excellent verbal and written communication and interpersonal skills as well as the ability to interact effectively at all levels of organization.


Specific qualifications for the role include:


•Registered Nurse (RN) with required current state licensure

•Minimum 3 years of clinical experience in a health care setting; experience in a long-term care setting, preferred

•Current/active CPR certification

•Firm knowledge of nursing and medical practices and procedures as well as laws, regulations, and guidelines that pertain to long-term care

•Willingness to learn and master the RAI Process in an effort to attain or maintain each patient’s highest practical level of function

•Proficiency in Microsoft Word, Excel, Power Point, and Outlook, Internet and Intranet navigation

•Commitment to the highest level of professionalism with the ability to maintain confidentiality

•Strong English language skills
