Preventative Health Outreach Coordinator
Provides telephonic and on-site outreach to members for preventive health services with a primary focus on the Texas Health Steps periodicity schedule for immunizations and well child visits. Participates in special community events, educational, preventive and wellness programs. Provides Member information, education and assistance in utilizing any Community Resource Services. Assists members with appointments, transportation coordination, and serves as a liaison between members, providers and interdepartmental areas, which will ensure that all contractual and /or regulatory requirements are met. Ensures compliance with the Health Insurance Portability and Accountability Act (HIPAA) related policies in regards to all aspects of operations within Member Services.
Minimum of two-year college or Associate degree related to health care administration or related work is required. Experience in community-based activities and customer service is required. Bilingual preferred. (English/Spanish) Minimum of two years supervisory preferred.
