The chaplain provides spiritual assessment and pastoral care plans for patients and their families, as well as crisis/on-call ministry particularly in times of spiritual distress brought on by life threatening or life altering medical diagnosis and procedures.

1 Master’s degree in Divinity or related theological field is required.
2 Minimum of four units of Clinical Pastoral Education is required.
1. Previous experience in a similar position is preferred.
1 Certification by the National Association of Catholic Chaplains (NACC), Association of Professional Chaplains (APC), or College of Pastoral Supervision and Psychotherapy (CPSP) is required.
KEY JOB DUTIES/RESPONSIBILITIES:
A. Assesses the spiritual needs of a patient/client. 15% (E)

B. Provides direct intervention or coordinates the care plan with other caregivers. 30% (E)
C. Documents the progress of the spiritual care plan in the patient record. 15% (E)
D. Provide crisis intervention ministry for patients/client and families. 20% (E)
E. Participates in inter-disciplinary care teams to assure continuity of care. 10% (M)
F. Provides consultation and education for associates in areas of spirituality and health. 10% (M)
G. Demonstrates competence to perform assigned responsibilities in a manner that meets the age-specific and developmental needs of the member served by the department. (E)
H. Appropriately adapts assigned assessment, treatment, and/or service methods to accommodate the unique physical, psychosocial, cultural, age specific and other developmental needs of each member served. (E)
