Nurse Practitioner - PRN - Optum CarePlus (Evercare) - Milwaukee

This job posting is for a PRN position in the Milwaukee area.

Our PRN position offers the opportunity to work alongside compassionate NPs to ensure continued care for our Senior members.



This is a PRN position that is as needed or as the situation arises and can be up to five days per week and is with Optum Care Plus, formerly known as Evercare.

In this role, you will provide primary care to patients in long term care settings; coordinating with their primary and facility staff to deliver high quality care on-site.

Key responsibilities of the NP include:



  • Provide preventative and primary care for patients in assigned long term care facilities.

  • Work with primary care physicians to provide the best care possible.

  • Collaborate with the nursing staff and the patients' families.

  • Work with the facility's utilization review process.

  • In this role, you'll also explore and learn more about palliative care. 

Responsible to cover team Nurse Practitioners in the event of FMLA, PTO and other business needs.



Key clinical responsibilities:



Complete monthly/quarterly progress notes and timely document in EMR.



Have ability to identify changes in condition and to put interventions to the plan of care to avoid preventable hospitalizations.



Be able to manage most common chronic conditions.



Have knowledge and possess ability/desire to work with geriatrics in long-term care settings.



Demonstrate learning ability to document acuity of member's condition in the EMR and desire to learn MA/HEDIS obligations.



Feel comfortable to have frank Advanced Care Plan discussions.



Work independently and yet in collaboration with staff in the nursing home, PCPs and other physicians.



Have strong ability to appreciate diversity and communicate changes in condition to families/members.


Knowledge of Microsoft office and ability to work with EMR.



Strong analytical, communication, leadership and problem solving skills.



Ability to multitask at any point of time, work with individuals of different backgrounds, needs and personalities.



Travel by car within designated areas in Wisconsin.



Flexibility in thought process (adoptability to change) and time- essential.

Job Requirements:

Education:


Master's degree in Nursing.


Certification by ANCC or ACNP as an FNP, ANP or GNP.


Current RN and APNP licenses in the state of Wisconsin or the ability to obtain.


Current DEA certification or the ability to obtain. 



Experience:


One year of hands-on post-grad clinical experience as an NP preferred


Knowledge of long-term care settings preferred


Strong ability to make independent clinical decisions.


Ability to adopt to new environments and people.
