Manager, Care Coordination
Under the direction of the Director of CARE, the Manager for Care Coordination will serve as a liaison with community resources and payors to promote organizational collaboration for the West Baltimore Health CARE. The Manager for Care Coordination will work with the individual Partners, Director of CARE, and other key member representatives to design, develop, implement, evaluate, and revise the delivery of care coordination services to ensure optimum patient outcomes with appropriate utilization of services throughout the partnership and community resources.
Minimum five years clinical experience (case management, primary care, chronic disease management,
psychosocial care, home health care) and three years experience performing concurrent review, discharge
planning, or case management.
Minimum three years in a supervisory or management role.
Experience in program development and project management.
Knowledge of regulations related to Medicaid, utilization review, discharge planning, and social services.
Knowledge and skills related to utilization management, patient advocacy, and optimal use of health care
