SERVICE COORDINATOR



POSITION DESCRIPTION:

Under direction, provides case management services to persons with developmental disabilities and their families. Provides case assessment and makes use of all community resources on behalf of agency consumers. Provides information and referral services.

With consultation from supervisor and/or other agency staff members, develops and implements appropriate case plans; obtains consumer services through referral to local resources or through the purchase of needed services. Provides follow-up contacts, reassessments, and periodic review of consumer's progress and program effectiveness. Acts as an advocate for consumer and assists families in obtaining and using needed services and programs. Represents agency at community meetings and may make public presentations regarding Regional Center services and activities. Will act as an informed advocate with issues related to special education and criminal justice. Additional duties may be assigned as deemed appropriate.

Job Requirements

  • BA or BS Degree, REQUIRED, from an accredited college, with an emphasis in social work, psychology or a closely related field

 

  • At least one year of applicable professional experience

 

  • MUST BE FLUENT IN MANDARIN/CANTONESE/ENGLISH
  • Valid California Driver's License & proof of insurance
