Medical Assistant - Stanislaus County

SUMMARY:

Under the direct supervision of the Medical Records Supervisor or Office Coordinator which ever is appropriate. The Medical Records Clerk is responsible for the maintenance, filing, and updating of all patient records.

1. Files all loose medical documentation in the appropriate chart: must ensure all paperwork is initialed by a provider before filing.

2. Pulls charts as requested for billing, medical, and dental. Replace chart with an Out Guide and Request Slip to assure speedy retrieval.

3. File all returned charts in the appropriate place according to the PFN number.

4. Pull medical charts for the next day's appointments and delivers to the appropriate area:

    a. Use a copy of the appointment log to obtain a list of patients and PFN numbers.

    b. Review charts being returned to medical records, to ensure the visit was documented  with provider’s signature and all loose paperwork is secure before filing chart.

5. Prepare and mail out Authorization for Release of Information as requested per policy.

6. If authorization to release medical records is signed, send a copy of the three page prenatal history plus all pertinent laboratory and supporting data to the appropriate hospital the first week of the month prior to the Expected Date of Delivery.

7. Performs purging function per approved policy.

8. Keep medical records room neat and tidy.

9. Assist in other positions as needed.

Requirements

QUALIFICATION REQUIREMENTS:
Ability to:

1. Ability to work well and concentrate in a busy office setting.

2. Ability to file (both alphabetically and numerically) with an outstanding degree of accuracy.

3. Ability to follow directions well without constant supervision.


Training and Experience:
1. High school graduate.

2. Previous typing/clerical experience preferable.

3. Aptitude for detail and precision.


PHYSICAL REQUIREMENTS:
1. Must be able to lift up to 40 pounds and push up to 100 pounds (on wheels).

2. Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.

3. Must have vision with or without lenses that are adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.

4. Must have high manual dexterity.

5. Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
