Medical Case Manager

General Description:  The Medical Case Manager coordinates resources and creates flexible, cost-effective options for catastrophically, chronically ill or injured individuals on a case-by-case basis to facilitate quality and individualized treatment goals including timely return to work.  Challenging opportunties for team-oriented individuals.

Several Full-Time, Part-Time positions available in the following locations:

                                                               Durham/Chapel Hill/Greensboro

Required Education/Training: 

• Current unencumbered NC Registered Nurse Licensure
• Minimum 3 years recent clinical experience in Orthopedics, Neurology or Rehabilitation
• Certifications:  CCM, CRRN, COHN or CDMS
• BSN preferred
• Computer proficient
• Superior communication skills (oral and written)

Essential Job Duties:

• Provide case management through coordination with the patient, physician, other health care providers, employers and the referral sources.
• May attend doctor/attorney visits, hospitals and/or long-term facility, discharge planning.
• Heavy travel required for all field positions.
• Minimum billable standards must be met along with reporting requirements.
• Conference attendance and additional duties as required.

Bi-Lingual Spanish skills a PLUS.
