HP's Enterprise Solutions Care Management Practice
In support of its leading position in the healthcare service industry, HP Enterprise Solutions has established the Care Management practice to help clients address the need for assessing,   planning, implementing, coordinating, monitoring and evaluating service and resource utilization needs of their membership while improving health outcomes.
 
As part of the Care Management practice this position will help design, develop and implement individual and population based strategic care management programs and clinical information systems that will deliver value-added solutions to clients. Working with the Care Management practice will provide team members with significant responsibility and growth opportunities as HP Enterprise Solutions greatly expands its capabilities and business in care management market.
 
Job Description:  
Under general direction and as part of team, participates in designing, developing, and implementing care management solutions in support of healthcare client business. As a Registered Nurse working with the Care Management practice team, duties will include:
 
  • Researching current clinical best practices in the medical literature;
  • Designing, developing, and implementing individual and population based care management programs;
  • Developing clinical information systems inputs;
  • Interpreting evidence-based treatment guidelines and assisting in the coding of clinical algorithms and business rules;
  • Support new business pursuits, via formal requests for proposal and ad hoc service quote requests

    Job Essential Functions: 
  • Ability to collect/analyze clinical information/data and apply findings to program design;  
  • Ability to define business process workflows necessary to implement designed programs;
  • Ability to identify information system inputs necessary for efficient program implementation;      
  • Ability to communicate effectively;  
  • Ability to support business studies and proposal teams;
  • Ability to work independently, proactively, and as part of team
     
  • Requirements Skills & qualifications: 
  • Registered Nurse with active non-restricted license;
  • CCM, CCMA, or  related AANC certification or eligible to sit for certification and willing to obtain certification within 3 months of hire;
  • Experience with and/or knowledge of clinical information systems;
  • Strong verbal and written communication skills;
  • Above average computer/technical skills;
  • Knowledge of medical coding including ICD-9 and CPT code sets desirable;
  • Knowledge of NCQA HEDIS metrics or other standardized performance metrics desirable;
