Home Health Aide HHA Certified Nursing Assistant CNA

Job Description Summary:

We are currently accepting applications for CNA’s for part-time schedules. In this role, you have the opportunity to work with one client, utilizing your clinical and exceptional customer service skills to help our clients remain in their homes.  You will work with one client in a single location for a 3 to 8 hour shift, depending on your availability.


The Home Health Aide/CNA is a certified paraprofessional member of the home care team who performs home care visits and works under the supervision of a Registered Nurse or Therapist. The Home Health Aide/CNA also performs various personal care services as necessary to meet the patient’s needs.  The Home Health Aide/CNA is responsible for observing patients, reporting these observations and documenting observations and care performed while performing the home care visit.


The Home Health Aide/CNA will be assigned in a manner that promotes quality, continuity and safety of a patient’s care.

**This position is for a Live-In Aide or 24 hour caseload**
Essential Job Functions/Responsibilities:

Under the direction of the Case Manager or designee the following personal care duties may be performed in assisting the client:

  • Personal hygiene
  • Ambulation
  • Eating, meal preparation
  • Dressing
  • Shaving
  • Toileting

Performs duties as taught by supervisor for a specific client.  These may include:    
  • Shampoo

Assisting with the use of devices required for daily living:
  • Walker or wheelchair
  • Prescribed ice cap or collar
  • Crutches or cane
  • Assists with prescribed range of motion exercises under the supervision of the individual(s) in charge of the case
  • As assigned, measures and prepares special diets
  • Record intake and output
  • Encourages and offers fluids to provide adequate oral intake
  • Records personal and therapeutic health care activities
  • Observes appearance and behavioral changes in the client and reports to the Supervisor.
  • Follows infection control standard precautions and safety policies per agency, state and federal regulations. Uses personal protective equipment as needed
  • Maintains clean and neat client environment
  • Assists client to meet socialization needs
  • Accompanies client to activities
  • According to state and federal guidelines handles clean, soiled and contaminated linen as instructed and properly disposes of waste material
  • HHA/CNA activity sheets and times slips are submitted to the agency weekly
  • Responds quickly and courteously to requests of the clients and heath care team

Performs assigned tasks in the following emergencies as described by policy:

  • Fires
  • Electrical Failures
  • Disasters, including severe weather
  • Maintains confidentiality of client information
  • As instructed, provides medication reminders
  • Read and record temperature, pulse, respirations and weight
  • Report all incidents/accidents to the agency immediately
  • Utilizes effective written/verbal communication in English
  • Follows established guidelines of communications and authority
  • Dependability:  attendance, tardiness
  • Other duties as assigned


  • Twelve (12) working hours per day, not necessarily in consecutive order
  • Eight (8) sleeping hours
  • Four (4) hours for self needs and personal care, not necessarily in consecutive order
  • Days off must be scheduled through the agency office

Job Requirements

Home Health Aide HHA Certified Nursing Assistant CNA

  • Certification as required by State regulations
  • Employment as a Nurse’s Aide within the past twenty four (24) months per regulations where required by state regulations
  • Must receive a passing score on the test administered per agency policies
  • Current CPR where required by state regulations
  • Ability to read, write and communicate in English
