Responsibilities:Provide chemical dependency recovery services including care, patient and family education; case management for assigned adult patients; oversee patient milieu to assure its safety and therapeutic integrity for assigned adult patients; and act as a member of the multidisciplinary team; facilitate and maintain communication with referents and other stakeholders in the patient's care.

Job Requirements

Bachelor's degree required; Master's degree desired; current IL IAODAPCA certification or LCPC or LCSW required; minimum 3 or more years experience in the CD field; 1 or more years providing IOP CD treatment desired; computer skills required.
