Provides therapy services per appropriate discipline (Physical, Occupational, or Speech Language Pathology) to patients referred, including assessment, treatment plan development and implementation with follow-up and discharge planning. Will assist the Director/Manager in direct oversight of daily patient care operations in outpatient and/or inpatient areas of the department. Will provide staff support, staff development, ensure appropriate patient care assignments, and assist in ensuring appropriate staffing patterns for caseload. This position will constitute 60% patient care/40% administrative duty.

A. Performs comprehensive diagnostic evaluations to determine the status of the physical and cognitive abilities of inpatients and outpatients, considering diagnosis and age specific needs.
B. Develops and implements, with patient/family participation, individualized treatment plans, as indicated, considering diagnosis and age specific needs, with measurable goals and methods to achieve goals.
C. Adheres consistently to the P&Ps relating to the documentation, to include: Initial Patient Evaluations, Daily and Weekly Progress, Notes as required, Discharge Summaries and other notations as Appropriate
D. Participates in patient care conferences, family conference, in-service programs, departmental and interdepartmental meetings.
E. Counsels both patient (if appropriate), family and other involved individuals regarding patient evaluation, care and treatment.
F. Initiates and completes discharge planning in a timely manner and provides effective education and training.
G. Assists Manager/Director with supervision of therapists, ancillary personnel, volunteers, and students.
H. Assists Manager/Director with Orientation of new Associates, staffing/patient scheduling to ensure productivity for all associates, staff development which may include coaching and/or mentoring.
I. Assists Manager/Director to ensure all associates meet assigned competency requirements.
J. Assists the Manager/Director in Performance Improvement activities.
K. Assists with organization/conducting department meetings; provides communication for Associates to ensure that all employees are kept up-to-date and that a team approach to providing rehabilitation services is maintained.
L. Maintains an awareness of outside agencies in the community to make appropriate referrals, when indicated, to network and increase knowledge of community resources.
M. Identifies and provides educational opportunities for self, department, and community. Participates in research activities as appropriate.
N. Fosters and promotes positive and professional relationships with members of the medical staff, physicians, and referral sources.
O. Demonstrates competence to perform assigned patient care responsibilities in a manner that meets the age-specific, developmental, and population specific needs of members served by the department.
P. Appropriately adapts assigned patient assessment, treatment, and/or care methods to accommodate the unique physical, psychological, cultural, age-specific and other developmental needs of each patient served.
Q. Takes personal responsibility to ensure compliance with all policies, procedures and standards as promulgated by state and federal agencies, the hospital, and other regulatory entities.
R. Performs all duties in a manner that protects the confidentiality of patients and does not solicit or disclose any confidential information unless it is necessary in the performance of assigned job duties.
