RN Transplant Coordinator-CTI Children's Hosp

Job Description

This position is responsible for providing advanced, professional, transplant nurse coordination to patients requiring Kidney Transplant.

A. The organ transplant coordinator provides care to patients using the nursing process as a guide of actions. (E)
B. Performs age appropriate psychological, environmental, nutritional, functional, educational and discharge planning assessment or referral for assessment of the patients with end stage organ disease on admission, (in house-outpatient) and throughout the episode of care. (E)
C. Provides advanced decision making and physical assessment skills. Performs nursing actions effectively based on annual competency assessments  and unit specific standards. (E)
D. Formulates an appropriate plan that addresses educational needs, discharge planning needs, age specific needs, bio/psychosocial needs and patients/family preferences. (E)
E. Provides for the emotional and physical comfort of patients. (E)
F. Provides patient/family education specific to the patient’s disease process, care processes/equipment, rehabilitation needs, medications, diet and lifestyle modifications. (E)
G. Evaluates the care delivered and updates the plan of care. (E)
H. The Transplant Coordinator employs effective clinical skills. (E)
I. Screens new potential candidates by reviewing medical records while working in conjunction with physicians. (E)
J. Establishes patient care priorities. (E)
K. Sets appointments and provides transplant specific information to new end stage organ disease candidates for organ transplant workups. (E)
L. Collects and follows-up on initial data including medical history and review of systems. (E)
M. Orders evaluation testing: lab/radiology/consults as required by organ transplant evaluation protocols and ordered by CTI physicians. Follows up with CTI physicians on abnormal clinical results. Record results in chart and forwards results/MD progress notes to referring physicians. (E)
N. Documentation is thorough, complete and timely. (E)
O. Ensures that patients are listed on the national transplant waiting list. Performs all duties as pertains to listing patients such as coordination of patient travel, and communicating such to the referring physicians. (E)
P. Provides inpatient/outpatient clinical follow-up after organ transplantation; triage patient needs and follows up with physician as necessary. (E)
Q. Interfaces with and coordinates efforts of admissions, business office, medical records, financial services, home care, rehabilitation, social services and risk management relevant to CTI cases. (E)
R. The Organ Transplant Coordinator employs effective leadership skills (E):
S. Plans, organizes and delegates to other healthcare personnel based on knowledge of patient needs and skills and competencies of the personnel. (E)
T. Copes with stress and change positively. (E)
U. Assists other associates to develop skills and knowledge. (E)
V. Participates in quality improvement projects. (E)
W. Identifies patient care quality concerns and brings this to the attention of supervisor. (E)
X. Assists in the development of CHRISTUS Transplant Institute’s sponsored educational programs and services for physicians, service line managers, system managers and the staff of CHRISTUS Healthcare facilities and ancillary providers. (E)
Y. Actively participates in the implementation of the systematic procedure/process/policy for all activities CTI provides, that facilitate CTI’s ability to meet the needs/concerns of physicians, department heads, and clients of CHRISTUS Healthcare. (E)
Z. The Organ Transplant Coordinator creates/maintains a therapeutic environment for patients and their families (E)
AA. Adheres to principles and policies of safety, risk management and infection control in nursing care implementation. (E)
BB. Creates a healing environment through minimizing voice, light, and other stimuli as appropriate for patients.(E)
CC. The Organ Transplant Coordinator participates in the marketing process of CHRISTUS transplantation services (E)
DD. Provides customer education on the transplant program and process to local and regional service areas. (E)
EE. Assesses service area for marketing needs and reports to marketing specialist and administrative director of transplantation services. (E)
FF. Demonstrates competence to perform assigned responsibilities in a manner that meets the population-specific and developmental needs of the members served by the department. (E)
GG. Appropriately adapts assigned assessment, treatment, and/or service, methods to accommodate the unique physical, psychosocial, cultural, age-specific and other developmental needs of each member served. (E)
HH. Demonstrates adherence to the CORE Values of CHRISTUS Santa Rosa Health Care. (E)

  • Job Requirements

    1. RN required.
    2. Bachelor’s Degree in Nursing is preferred.

    1. Requires a minimum of five years RN nursing experience, preferably in the areas of transplant, or critical care.

    1. Current RN License is required.
    2. CPR is required.
    3. CCTC is preferred.

    1. Requires ability to work independently.
    2. Requires in depth knowledge of the nursing process.
    3. Requires specialized knowledge of pathophysiology and clinical therapeutics related to a selected category of patients.
    4. Requires a thorough knowledge and understanding of the impact of illness on the psychosocial and developmental needs of patients and families.
    5. Requires a working knowledge of group dynamics.
    6. Requires strong written and verbal communication skills.
    7. Requires the ability to perform basic and advanced skills and utilize equipment required for specialty area assigned as outlined on the skills checklist.

    1. Job may include travel, & if so, must be willing to travel to outreach facilities which may also involve overnight travel.
    2. Works in a smoke free, drug free facility.
    3. Must adjust to frequently changing workloads and constant interruptions.
    4. Constant walking and/or standing is necessary.
    5. Some physical stamina is required to lift and move patients.
    6. Subject to strained muscles and possible exposure to body fluids and infectious disease.
    7. Takes call as agreed upon by schedule. All on call time includes calls regarding potential donors and their management, calling the recipients in for the potential transplant, coming into the medical center to verify patient’s paperwork and any or all illness calls which occur after business hours weekdays, weekends, or holidays to ensure continuity of care.
