Health Care Coordinator Community

RN Required

General Responsibilities

The Health Care Coordinator gives direct supervision and coordination for all the medical services and health care needs of the individuals receiving services at the Melmark.


Essential Duties          

  1. Supervises the shift nurses. Completes 90 day and annual evaluations for nurses. Provides corrective action to nurses when necessary.
  2. Communicates actively and professionally with parents at all times.
  3. Provides comprehensive admission and discharge health care information.
  4. Coordinates with the Primary Care Physician (PCP) for proper medical care and wellness of all individuals. 
  5. Supervises and takes responsibility for all nursing tasks including medication administration (training and oversight) and all medical aspects.  Provides in-services for all staff and individuals regarding health care issues.
  6. Ensures all required medical paperwork is completed (90-day reviews, Utilization Reviews, reports, etc.).  Provides regular reviews of medical records (appointments, lab results and current proper diagnosis). 
  7. Responsible for the completion of medical appointments (setting up appointments, posted weekly appointment schedules) and completion of medical summaries from appointments.  Frequently interfaces with doctors, nurses and practitioners.

Job Requirements


Education/Training:   Registered (RN) Nurse preferred. 

Certifications/Licensure:  Must hold a current Pennsylvania Registered Nursing license.  Successful completion of employer provided certification in CPR, First Aid, Medication Administration and Crisis Intervention.  Must have held a US Driver's License for a minimum of two years and must be able to meet requirements to become an approved Melmark driver.  Must be at least twenty-one (21) years old.   Current CPR and First Aid certification and 24 hours of in-service training yearly.

Skill(s):  Knowledge of regulatory standards for PA's DPW 6400 Regulations.  Working knowledge of Windows Operating Systems and Microsoft Office.  Should demonstrate strong interpersonal skills, good writing, communication and organizational skills.

Experience:  A minimum of two (2) year experience in related field preferred.
