You'll have the choice of 3 different PPO plans designed to provide protection to you and your immediate family. All plans are generously subsidized by STHS. Dental Insurance Care is divided into 4 basic categories: preventative, basic restorative, major restorative and orthodontia. All plans are generously subsidized by STHS. Vision Insurance Coverage allows benefits for yearly eye exams, eyeglasses, contacts and discounts on laser vision care by utilizing preferred providers in the plan. The "hard to fill" positions may be modified, as needed, at STHS' discretion. STHS reimburses up to $1000 for other programs that contribute to the growth and development of the Associate, as well as, STHS. Eligibility after 90-days employment, with a two-year agreement for degreed programs and a two-year work agreement for certificate programs agreement after graduation. Bonus Programs Employee Referral Program* Allows associates to earn a financial bonus for referring candidates for employment. Annual Performance Based Merit Increase Wellness: A Choice!* An incentive program to encourage healthier lifestyles. Total Compensation Statement Personalized and confidential summary that shows you the total value of your benefits and income. Protection & Insurance Coverage Short-Term Disability (STD) and Long-Term Disability (LTD) Coverage begins after 180 days of employment for full-time status associates only. STHS pays 100% of the cost, giving you 50% of pay during short-term and long-term illness or disabilities. You may purchase additional STD or LTD for 60% or 70% of pay protection in either plan. Basic Life and AD&D Insurance One-times your annual base pay in Life and AD&D Insurance provided by STHS. Spouse and Child Life Insurance Coverage for your spouse offered in amounts ranging from $10,000 - $100,000. Coverage for children in amounts of $5,000 and $10,000 per child. Supplemental and Voluntary AD&D Insurance You may purchase one-to-five-times your annual base pay in additional term life insurance. Coverage is available from $10,000 to $1,000,000 for associates and their family. Voluntary Whole Life Individually owned and portable life insurance plan that allows you to build cash value over time offered through Innotech Benefit Solutions. Critical Illness Insurance Financial protection on top of your health plan and disability in case of certain serious health problems provided through Innotech Benefit Solutions. Long-Term Care * Not available at all campuses
