Overview


Plan de Salud del Valle, Inc., a 501(c)(3) nonprofit corporation, also known as Salud Family Health Centers (Salud) was established in 1970 to meet the health care needs of farmworkers and their families in north central Colorado. In its 36-year history, Salud has evolved into a primary health care delivery system providing services at 14 clinics in nine communities which cover six northcentral/ northeastern counties. Salud focuses on the needs of the medically indigent, the uninsured, and the under-insured. The U.S. Public Health Service has designated Salud as a Federally Qualified Migrant and Community Health Center.

The Salud system provides a full spectrum of high-quality, comprehensive, culturally competent, primary medical and dental care, including obstetrics and out-patient care to more than 66,000 residents and migrant and seasonal farmworkers in northcentral and northeast Colorado. Salud also operates a mobile clinic, which serves to increase access to health care by migrant and seasonal farmworkers. Salud currently provides more than 225,000 patient visits. Salud is also a significant employer in northern Colorado with nearly 400 employees.

Culture

Salud's ultimate goal is to provide quality, comprehensive primary health care services, and to improve the overall health of the communities it serves by reducing barriers to health care, including: ability to pay, transportation, and language. Salud provides health care services without regard to age, sex or disease process. Salud's clinics provide an important safety net function within the communities they serve. The majority of Salud's clients have little or no access to other health care providers due to a lack of private insurance. Salud has a firm commitment to provide care to all people, and does not turn patients away based on finances, insurance coverage, or ability to pay. Mission Statement Plan de Salud del Valle, Inc. (Salud) is a health care delivery system dedicated to providing quality, comprehensive primary health services to all the residents of a defined catchment area, covering parts of Weld, Boulder, Adams, Larimer, Morgan and Logan Counties, Colorado.

Medical Services

Salud offers a full spectrum of primary care services at its family practice, adolescent and pediatric clinics including:

  • Family Medical Practice - management of acute and chronic illnesses
  • Pediatric & Adolescent Care
  • Minor Surgery
  • Pregnancy Care and Delivery
  • Physical Exams (Well Child Checks, Sports, Schools, INS, DOT)
  • Family Planning and Gynecological Services

Dental Services
A full range of dental services are available at each of the four dental clinics in the Salud system:

Women's Health Services

All of Salud's Family Practice clinics offer pregnancy care and delivery and family planning and gynecological services. In addition, the Brighton Women's Center at our Salud Family Health Center in Brighton offers:

Pediatric Services

Each of the Salud family practice clinics in the system serve adults and children. In addition, there are three Salud Clinics that provide specialized Pediatric Care staffed by pediatricians:

  • Salud Family Health Center, Fort Lupton, Pediatric Clinic
  • Salud Family Health Center, Longmont, Pediatric Clinic
  • Salud Family Health Center, Fort Collins, Children's Clinic


Services provided by the Pediatric clinics include:

  • Complete care of patients from birth to age 18
  • All services tailored to children's needs
  • Experienced, bilingual Pediatrician
  • Well-baby check-ups
  • Immunizations

Adolescent Services

    There are three specialized adolescent clinics within the Salud system: the "Higher Grounds" Clinic at the Salud Family Health Center in Commerce City, and "New Horizons" at the Salud Family Health Center in Brighton and the evening Young Adult and Teen Clinic (YATC), at the Salud Family Health Center in Fort Collins. Services offered at these clinics include:

  • Serving the adolescent ages 12 to 20 (11 to 21 in Ft. Collins)
  • Family Planning & Gynecological Services
  • School Physicals
  • Confidential visits
  • Pregnancy care & delivery
  • Peer counselors in social work
