Job Description

Head Injury Association is seeking a dynamic Admissions Coordinator to join their team. The responsibilities for the positions are as follows:

The Admissions Coordinator will provide intake services for all referrals into all TBI, NHTD, and OPWDD Waiver Programs such as Day Habilitation, Residential Habilitation, Service Coordination. The Admissions coordinator will ensures that consumer program vacancies are filled to capacity as assessed through review of reports to supervisor. They will establish waiting lists of pre-screened and accepted individuals for every program. The Admissions coordinator will review admission packages for appropriateness as assessed through supervisory review and observation.

They will also schedule placement interviews/screenings for relevant Program management.

Coordinates the Admissions Committee Meeting on an as needed basis and make placement recommendations based upon review of screening information and interviews, program availability and waiver eligibility as assessed through supervisory review.

Also, the Admissions Coordinator will participate in grant/contract proposal development and also responsible to respond to all RFPs as directed.


Job Requirements

  • Bachelor’s Degree in Social  Work or related field. 
  • Must have and maintain a valid NYS Driver’s License with a satisfactory driving record.
  • 2 years of Service Coordination experience in the TBI or OPWDD Waiver field or related field required.
