Job Description

General Summary:
The Quality Improvement (QI) Coordinator (I) will work under the direction of the Director, Quality Improvement to provide support with planning, organizing, and coordinating quality improvement activities and projects throughout the organization. The QI Coordinator will work independently as well as have responsibilities to work in collaboration with our team to support quality improvement initiatives including practice coaching, data management and reporting, health insurance incentive programs, population health management and other value-based reimbursement programs.


Duties and Responsibilities:
• Support the health system goals to provide quality health care to all of our patients
• Act as a valuable resource and contact person for our practice sites
• Assist in the communication and coordination of health insurance incentive programs
• Work in partnership with the Director of Quality Improvement to identify and/or development resources to aid the organization with quality improvement projects
• Represent for the Quality Improvement Department at appropriate meetings, seminars and conferences
• Maintain and distribute routine reports to all members of our team to track progress in our quality improvement programs throughout the calendar year
• Perform routine tasks related to health plan data management, including monitoring and maintaining data integrity
• Prioritize, anticipate and assess quality improvement needs for our practice sites
• Assists in the delivery of educational programs for providers and office staff
• Acts as a bridge for information exchange between practice sites and the corporate office
• Travels from corporate to RWJPE practice sites within New Jersey
• Other duties as assigned

Job Requirements

Educational and Experience:
• Clinical background, required.
• Minimum of 2 years of experience working in healthcare setting required
• Population Health Management and/or Care Coordination Experience preferred
• Proficiency with Microsoft Excel and Word and PowerPoint
• Strong multi-tasking abilities


Knowledge, Skills and Abilities:
• Knowledge of basic principles related to Population Health, ACO, Care Coordination and Quality Based Payment Models
• Knowledge of Electronic Health Record navigation
• Demonstrates and maintains a high level of accuracy and attention to detail
• Strong ability to work independently
• Ability to work effectively with diverse populations both internally and externally
• Demonstrates awareness and value of cultural competence
• Excellent writing, research, analytical and time management skills
