Home Health Occupational Therapist (Per Diem)


POSITION SUMMARY

Provides services to home health clients in accordance with the Plan of Care. Services are provided under the direction of the attending physician with participation of the family and other members of the interdisciplinary team, as indicated.

ESSENTIAL FUNCTIONS/ AREAS OF ACCOUNTABILITY
  1. Performs occupational therapy assessments, diagnostic tests, skilled treatments, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care.


  • Completes assessments in a timely manner and in accordance with agency policy.
  • Develops an occupational therapy plan to provide direction to other health team members involved in the Plan of Care.
  • Reviews and updates the care plan based on client need and agency requirements.
  • Documents all findings, plans, interventions, and client progress (outcomes) in the clinical record.
  • Communicates plans and changes to the physician and to the client’s nursing Case Manager and other agency staff through the care plan, written progress notes, and participation in care conferences.
  • Performs Medicare therapy reassessments on active cases every thirty (30) days or sooner as dictated by Case Manager or agency policy.
  1. Participates in the development of the Plan of Care.
  2. Guides and instructs the client in prescribing therapeutic self-care and creative activities that are directed toward improving independence and physical and cognitive functioning.
  3. Prepares a report of each client’s diagnostic evaluation and makes recommendations.
  4. Prepares clinical and progress notes.
  5. Advises, consults, and teaches appropriate treatment procedures and supportive activities to clients, family members, and other agency staff.
  6. Establishes household management routines.
  7. Designs and/ or orders appropriate adaptive/assistive devices and/or splints to promote optimal level of functioning.
  8. Records and reports to he physician the client’s reaction to the therapy program or any changes.
  9. Instructs clients and their families in Activities of Daily Living.
  10. Instructs the family in the client’s total occupational therapy program.
  11. Instructs other health personnel regarding occupational therapy for specific clients.
  12. Attends care conferences as deemed necessary.
  13. Participates in in-service programs.
  14. Serves on the Clinical Record Review Committee.
  15. Performs other related duties and responsibilities as deemed necessary.
  16. Performs in-home supervision and/or supervisory visits of home health aides and according to regulations and agency policy.

 

SUPERVISION OF OCCUPATIONAL THERAPY-ASSISTANTS

  1. Instructs and supervises Occupational Therapy-Assistants in certain phases of occupational therapy services they may provide to the client.
  2. Available by telephone to the Occupational Therapy-Assistant al all times when an Occupational Therapy-Assistant is providing treatment to a client.
  3. Completes an on-site assessment, review, and revision of the Plan of Care, and an assessment of utilization of community resources as needed.
ENVIRONMENTAL ADAPTABILITY

  • Works primarily indoors in a climate controlled setting.
  • Team Members may be exposed to blood and/or body fluids with potential exposure to hazardous materials and infectious diseases. 
  • Possible exposure to unpleasant odors.
  • Possible exposure to chemicals as identified in the MSDS Manual.
  • Continuous exposure to residents who are ill, confused, irritable and irrational.
PHYSICAL REQUIREMENTS

While performing the duties of this Job, the employee is regularly required to stand. The employee is frequently required to walk; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is occasionally required to stoop, kneel, crouch, or crawl. The employee must frequently lift and/or move up to 50 pounds independently and up to 190 pounds with assistance of a second employee and/or mechanical devices.  Must be able to push up to 350 lb. (wheelchair). The employee may occasionally be required to lift greater than 190 pounds with the assistance of additional staff and/or a mechanical lift. Specific vision abilities required by this job include close vision and distance vision.

Job Requirements

QUALIFICATIONS

  1. Graduate of an accredited occupational therapy curriculum, accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; or eligible for the National Registration Examination of the American Occupational Therapy Association.
  2. Current registration or license as required by state(s) of practice.
  3. Minimum of one (1) year’s experience in a health care facility and Home Health experience preferred.
  4. Knowledgeable in principles and practice of Occupational Therapy.
  5. CPR certification.
  6. Strong verbal and written communication skills and good interpersonal skills.
  7. Current driver’s license, safe driving record, and reliable transportation.
