Job Description

SCOPE:

The Health Care Navigator will educate program participants about the availability of qualified health plans and provide fair and impartial information about enrollment in qualified plans and about the availability of tax credits and financial assistance available in the Health Insurance Marketplace. Navigators will assist program participants by helping them to enroll in qualified plans including completing and submitting eligibility and enrollment forms with the participant. The Health Care Navigator assists program participants who are experiencing a problem with their health plan or with their premium assistance by making a referral to a consumer assistance or ombudsman program or to another appropriate agency.

ESSENTIAL FUNCTIONS:

  1. Perform pre-admission health insurance/Medicaid coverage and eligibility assessments.
  1. Educate program participants about the availability of qualified health plans.
  1. Provide information about enrollment in qualified plans and about the availability of tax credits and financial assistance available in the Health Insurance Marketplace.
  1. Assist program participants by helping them to enroll in qualified plans including completing and submitting eligibility and enrollment forms with the participant.
  1. Assists program participants who are experiencing a problem with their health plan or with their premium assistance by making a referral to a consumer assistance or ombudsman program or to another appropriate agency.
  1. Counsel client family members to assist them in dealing with and providing support for the client.
  1. Refer clients or families to other support services as appropriate and within the parameters of designated program, such as medical evaluation and treatment, social services and employment services and prepare and maintain required documentation for chain of custody.
  1. Complete required forms and make all necessary documentation to client files relating to all counseling content, observations, activities and issues relating to each client contacted. Prepare periodic and special reports relating to work activities and project progress. Prepare documents for presentation in court and accompany client to court as required. Participate in or lead projects involved in program planning and development or curriculum. Perform or manage crisis interventions as required.
  1. Demonstrate expertise and knowledge in the application of appropriate modalities, methodologies, approaches and techniques.
  1. Maintain high standards of ethical and professional conduct and adheres to Agency policies and procedures.


  • Level II Background clearance
  • Acceptable Motor Vehicle Registration driver's license record in accordance with the underwriting guidelines set by Aspire Health Partners' insurance company.
  • 2 year of experience in Behavioral health and terminology.
  • High school diploma or general equivalency degree with preference given to individuals with a Bachelor's Degree in Social Work, Healthcare or other related Human Services field.
  • Preferred LPN license in good standing with the State of Florida
