SENIOR INDEPENDENCE RECEIVES GRANT FROM THE CLEVELAND FOUNDATION


Cleveland , Ohio – Senior Independence, the home and community based services division of Ohio Presbyterian Retirement Services (OPRS), has been awarded a $100,000 grant in support of a new and innovative Diabetes Management and Prevention program to benefit the senior population in Lake and Cuyahoga counties.

Since 1922, OPRS's continuing care retirement communities and programs have defined the highest standards of quality of life for older adults. Each year, OPRS serves more than 4,500 residents in its 11 retirement communities and over 45,000 people though the Senior Independence program.

Senior Independence provides the necessary services to older adults and their families to enable staying in their own home for as long and as safely as possible. Launched as a local outreach program at Breckenridge Village in 1982, Senior Independence was the first program of its kind in Ohio . Today, Senior Independence is available in 29 counties, which serve the population base for

70 percent of Ohioans 65 years and older. It has been lauded as a model of community-based care, helping many older adults avoid premature and unnecessary admission into a nursing facility.

The grant from The Cleveland Foundation for $75,000 along with a $25,000 grant provided by its Lake Geauga Fund, will provide the resources for a comprehensive program incorporating the use of interactive telehealth in various types of senior housing. The telehealth system features live audio and video, combined with a telephonic stethoscope and other medical peripherals (blood pressure meter, scale, pulse oximeter, glucometer) to allow a nurse to conduct a “video visit” with clients in their homes, senior apartment complex or at designated senior centers in the Cleveland area. The nurse and client can see and talk to each other while the nurse gathers pertinent health information, allowing a timely clinical assessment of patient status without the need to travel to the doctor's office.

Telehealth provides clinical staff to assess, intervene and manage seniors who have been diagnosed or are at high risk for type 2 diabetes. If a problem is identified, the nurse can intervene rapidly and appropriate action can be taken immediately. Lois Calderwood, community services director for Senior Independence located in Willoughby , says “These grants provide a wonderful opportunity for Senior Independence in the Cleveland area to demonstrate the effectiveness of telehealth. It also provides the opportunity to serve large groups of seniors in a new and effective way.”
