Case Manager - RN NB

 Description About This Position

The Case Manager"s (CM) responsibility is to collaborate with the healthcare team to plan and manage the care of select patients from pre-admission to discharge. The CM will collect data, monitor and evaluate quality of care and resource utilization specific to agreed upon patient outcomes. The CM will facilitate quality and resource management in a team environment including Nursing, Medical Staff, and other healthcare disciplines as appropriate through concurrent quality reviews and clinical pathways. Bachelor of Science in Nursing preferred. A minimum of two years utilization review, discharge planning, case management, quality assurance, or quality related field or two years recent experience in Nursing/ Hospital Management in an acute care hospital. Has knowledge of and can apply the principles of managed care. Knowledge of payers, insurance, reimbursement, Medi-Cal and Medicare regulations required. Current California license as a registered nurse.
