Purpose of Job:  The Manager of Case Management will manage the TN Long Term Care Choices care management team, including RNs and SWs, with responsibility for clinical operations and medical management activities across the continuum of care.  Also, in collaboration with the Health Services Director, the Manager will work to ensure compliance with contract requirements as it relates to care coordination and related activities.
 
We are searching for multiple Managers to work with our teams in the following counties in the Middle Tennessee area:  Smith, Putnam, Macon, Jackson, Trousdale, Clay, Pickett, Overton, Fentress, Dekalb, Cannon, Warren, Van Buren, White, Cumberland, Wilson, Davidson, Sumner, Lawrence, Giles, Wayne, Marshall, Rutherford, Lincoln, Moore, Bedford, Coffee, Montgomery, Houston, Stewart, Robertson, Dickson, Cheatham, Hickman, Humphreys, Perry, Maury, Lewis and Williamson.
 
Managers can be based in our office in Brentwood, or may work from home depending on location.
 
Major Responsibilities:
 

  • Manages workload and balance of members assigned to each Care Coordinator to ensure they are within Tennessee-required ratios, and makes adjustments as needed
  • Sets team direction, resolves problems and provides guidance to members of own team
  • Develops, implements and monitors developmental plans for Care Coordinators in collaboration with other Case Management managers to implement Care Coordination model
  • Reviews all relevant operational reports ; i.e. assessment completions to identify areas of deficiency and implement strategies to address gaps in collaboration with Health Services Director
  • May oversee work activities of other supervisors
  • Adapts departmental plans and priorities to address business and operational challenges
  • Influences or provides input to forecasting and planning activities

Job Requirements

Required Qualifications:
 
  • Current, unrestricted RN or SW license required
  • Minimum of 1 year of management experience
  • Excellent organization, communication, and time management skills
  • Strong team player and participant in team building
  • Excellent conflict management, care influencing, and collaboration skills
  • Ability to proactively partner with care team, including physician engagement
  • Strong interpersonal skills
  • Strong problem solving and decision making abilities
  • Proficient computer skills including the ability to type and talk at the same time and toggle between multiple screens
 
Desired Qualifications:
 
  • Multilingual (English, Spanish)
  • Working knowledge of Medicaid/Medicare regulations
  • Working knowledge of medical terminology
  • Experience in chronic disease management
  • Long term care experience
  • Experience in case management to include assessment, planning, and service coordination
  • Highly desirable to have competency in Microsoft Word and Excel
  • Undergraduate or higher level degree
