Registered Nurse – RN – Social Work – Care Manager

As a Care Manager at Long Term Care Group, you will work with enrollees to provide continuous assessment of benefits and appropriateness of care regarding their long-term care policies.  Care Managers are critical in supporting our clients continued growing needs.

Responsibilities:

• Reach appropriate and accurate benefit eligibility decisions for claimants based upon policy provisions, proposed care plans, and assessment of clinical data.
• Work with care management agencies, beneficiaries, and claimants over the phone
• Answer calls, questions, and type notes simultaneously in a fast-paced environment.  

• Current, valid and unrestricted Registered Nurse – RN or Social Work - SW license in the state of Tennessee

• Minimum of an Associate’s Degree for Registered Nurse

• Minimum of a Masters Degree for Social Work

• 3+ years work experience preferably with the older adult population in a community setting or long-term care facility

• Previous telephonic or customer service experience

• Microsoft Office Suite
