Critical Care Pharmacist 

Critical Care Clinical Specialist Pharmacist

The Pharmacist is responsible for interpreting and processing medication orders, entering orders into the patient profile, and preparing and dispensing medications.  He / she will assist physicians in dose calculation and monitoring of drug therapies, as well as advise physicians, nurses, and others of medication information to assure their safe and effective use in individual patients.


General Responsibilities:

  • Accurately process medication orders; review for appropriate therapy, drug, dose, route, duration, and drug interactions. Communicate with prescribing physician to resolve problems and document therapeutic interventions.
  • Supervise technicians.
  • Monitor ongoing therapy for continued appropriateness of drug, dose, route, and duration. Recommend modifications of therapy when necessary based on age, renal function, height/weight, medical condition, etc.
  • Provide pharmacokinetic dosing and monitoring of aminoglycosides and vancomycin. Provide written (and verbal when necessary) dosing recommendations for initiation of therapy and subsequent to all serum level results.
  • Provides prospective drug use evaluation and offers medication counseling for all outpatient and discharge prescriptions dispensed.
  • Serve as liaison to nursing units or departments which use/or stock medications. Responsible for establishing inventory and par levels consistent with patient care needs and completion of monthly floor inspections.

Current MS Pharmacist License or eligible

Requires one year of Critical Care experience and/or of hospital based clinical pharmacist experience in the critical care setting. Prefer completion of a pharmacy residency program in critical care.
