Clinton Memorial Hospital, an affiliate of Sparrow Health System, is seeking a part-time, benefit eligible, Physical Therapist.

Primary Duties and Responsibilities:

1.       Evaluate and assess patients based upon the referral.  Establish reasonable goals with patient and/or family.

2.       Provide patient and/or caregivers education about the results of the evaluation, method of treatment and the results of treatment.  Instruct patients and/or family members in home programs.

3.       Perform treatment according to the established plan, utilizing all available modalities, equipment and techniques within the scope of physical therapy.

4.       Performs medical record documentation according to established criteria.

5.       Re-evaluate patients, assesses their progress, and make recommendations as to their discharge planning.

6.       Communicate with other health care professionals, including patient referral sources, verbally and in writing.

7.       Participate in inservices, staff meetings, team meetings, continuing education courses, and other meetings as needed.

8.       Lead or direct the activities of physical therapist assistants, aides, students and volunteers, ensuring that department policies and procedures are enforced and that quality patient care is provided.

9.       Provides inpatient care at Clinton Memorial Hospital based upon the referral.

10.   Demonstrates knowledge and respects patient, service provider, organizational confidentiality and HIPAA Security procedures and protocols as defined under the HIPAA Privacy and Security Procedures.

11.   Follows established HIPAA privacy procedures when using and/or disclosing protected health information.

12.   Maintains and protects patients rights under the HIPAA Privacy Standards.
