Registered Nurse Case Manager / Visiting RN for Home Health

Position: Registered Nurse (RN) Case Manager for Home Health patient visits. We have both a Part time per visit position and PRN opportunities for RNs. We are growing.

The RN position entails making visits to homebound patients for our growing Home Health Care Agency.

Job Summary:

·                          The Registered Nurse is responsible for the clinical care of assigned home care patients which may include but not be limited to case management of the case, specific nursing tasks, patient assessment, patient / family education, and coordination with the multidisciplinary team including RN, LPN, PT, PTA, OT, COTA, SLP, MSW and Home Health Aides.

·                          The RN is responsible to report changes in condition to the physician and to the Clinical Coordinator as appropriate.

·                          The position reports to the Clinical Coordinator.

What is Home Health?

·                          Nursing and Therapy team members visit patients in their homes on an intermittent basis according to doctor’s orders.

·                          Our skilled professionals work one on one with patients, caregivers and family members.

·                          Home Health allows health care professionals to utilize all of their accumulated knowledge and skills to help individual patients reach their goals as part of a multi-disciplinary care team.

·                          Clinicians perform 5-7 patient visits daily on average.

Education and Experience Summary:  

·  Must possess current RN license in good standing in the state in which agency is located.

·  Home Health experience preferred

·  Minimum of 1 year experience as a RN

·  OASIS knowledge preferred

·  Must have CPR certification

·  Valid driver’s license and auto liability insurance with reliable transportation to travel to patient homes per patient schedule and agency needs.
